【摘要】 目的:研究后程加速超分割放射治疗食管癌的疗效。方法:1995年5月~1999年6月,我院治疗食管癌患者240例,随机分为两组。常规分割放疗(CFR)组120例,每天1次,每次2Gy,每周5次,总剂量66~70Gy;后程加速超分割放射治疗(LCAHR)组120例,常规分割放射治疗30Gy后改为每天2次,每次1.5Gy,每次间隔6小时,每周10次,总肿瘤剂量63~66Gy。结果:LCAHR组和CFR组l3、、5年生存率分别为85.0%、45.0%、29.2%和62.5%、26.7%、16.7%(x2=14.55,P<0.01;x2=7.99,P<0.01;x2=4.62,P<0.05)。LCAHR组和CFR组1、3、5年局部控制率分别为70.0%、45.0%、40.8%和54.2%、29.1%、20.8%,两组差异有显著性(x2=5.73,P<0.05;x2=4.57,P<0.05;x2=10.34,P<0.01)。结论:后程加速超分割放射治疗有望提高食管癌的疗效;局部肿瘤复发和未控仍是失败的主要原因;是否提高放射治疗剂量有待于进一步临床研究。更多还原

【Abstract】 Objective:TO evaluate the effect of late course accelerated hyperactionalion radiotherapy(LCAH) in trement of esophageal carcinoma.Methods:From May 1995 to June 1999,240 patients with esophageal carcinoma were randomized into two groups,late course accelerated hyperfractionation radiotherapy group 120 patients and conventional fractionation radiotherapy group(CFR) 120 patienas.patients in CFR received 2Gy per fraction,5 fractions perweek with the total dose of 66～70Gy;Patients in LCAHR received 1.5Gy per fraction and two fractions per day with 6-hour interval,10 fractions per week with the total dose of 63～66Gy after 30Gy of conventional fractionation radiotherapy.Results:1-,3-and 5-year survival rates and 1-,3-and 5-year local control rates in LACHR group were significantly higher than those in CFR group(x~2=14.55,P<0.01;x~2=7.99,P<0.01;x~2=4.62,P<0.05),being respectively 85.0%,45.0%,29.2%vs62.5%,26.7%,17.7%;70.0%,45.0%,40.8%vs54.2%,29.1%,20.80%(x~2=5.73,P<0.05;x~2=4.57,P<0.05;x~2=10.34,P<0.01).Conclusions:Late Course accelerated hyperationation radiotherapy(LCAHR) may improve the treatment results in esophageal carcinoma.Local failure or uncontrolled lesions are still the main caurse of failure in LCAHR.Total dose higher than 63～66Gy in LCAHR group is suggested and warrants further study.更多还原
