远端蒂耳大神经营养血管筋膜皮瓣的应用解剖与临床应用

【摘要】 目的:探讨远端蒂耳大神经营养血管筋膜皮瓣修复口腔颌面部软组织缺损的临床应用。方法:在5例(10侧)灌注红色乳胶标本上解剖观察耳大神经营养血管筋膜皮瓣的血供。对4例颌面部肿瘤手术后的软组织缺损应用远端蒂耳大神经营养血管筋膜皮瓣修复,评价其临床效果。结果:4例皮瓣全部成活,其中1例远端约1/4的皮肤坏死,随访3个月至1年,外形满意、语言及咀嚼功能良好。结论:远端蒂耳大神经营养血管皮瓣血供可靠,可应用于口腔颌面部软组织缺损的修复。

【Abstract】 Objective: To explore clinical application of distally based greater auricular neurocutaneous fascia flap for repairing the soft tissue defects in oromaxillo-facial region. Methods: The blood supply and distribution of greater auricular neurocutaneous fascia flap were observed on 5 specimens (10 sides) perfused with red latex. 4 patients suffered from postoperative soft tissue defects in the oralmaxllo-facial regions were treated with distally based greater auricular neurocutaneous fascia flap and its clinical effects were evaluated. Results: All flaps survived except for partial dermal necrosis in one case. Followed up one month to one year, all patients were satisfied with the exterior, lingual function and masticatory function. Conclusions: The flap has reliable blood supply and can be designed to reconstruct the soft tissue defects in head and neck.
