自体半月板移植固定方法的实验研究

【摘要】 目的:探索半月板移植的固定方法。方法:30只成年大白兔随机分成3组,每组10只。将所有动物双膝关节内侧半月板切除,再以3种不同的固定方法回植入膝关节内。A组做前后角固定,B组做前后角+周边固定,C组做周边部固定。术后12周取材,进行大体和组织学观察,并测量半月板退变程度。结果:3组半月板周边均与关节囊愈合,A组中有2只半月板后角未愈合,B组有1只半月板前角未愈合,C组中仅有3只半月板的前角愈合。测量半月板退变结果显示:A组与B组无显著性差异;A组与C组、B组与C组有显著性差异。结论:在兔半月板移植实验中,半月板周边部是否固定对移植效果无明显影响,关键是前后角牢固固定。

【Abstract】 Objective: To study fixation methods of meniscal transplantantion. Methods: Thirty adult rabbits were randomly divided into three groups averagely. Medial meniscectomies were performed in all knee joints, and the menisci were replaced with three different methods. Anterior and posterior horns of the menisci were only fixated in group A; anterior, posterior horns and periphery were fixated in group B and the menisci were only fixated peripherally in group C. The autografts were excised and studied by macroscopic, histological methods 12 weeks after operation, and the degree of meniscal degeneration was measured. Results: At 12th week all grafts periphery healed up with articular capsule, but in group A posterior horns didn’t heal in two cases; in group B anterior horn didn’t heal in one and in group C no horn healed except three anterior horns. The meniscal measurement showed that difference between group A and B was not significant, but there were significant difference between group A and C, group B and C. Conclusion: The key point of fixation for meniscal transplantantion in rabbit was to fix the anterior and posterior horns of menisci, the peripheral part fixation of meniscus was not closely related to the effect of the procedure.
