肥胖高血压与非肥胖高血压患者血清磷脂脂肪酸谱的比较研究

【摘要】 目的测定肥胖高血压、非肥胖高血压患者与健康人的血清磷脂脂肪酸谱(作为生物标记物),研究高血压患者血清磷脂脂肪酸谱与血脂的相关性。方法抽取41例肥胖高血压患者、44例非肥胖高血压患者和53例健康对照者,采用高效气相色谱法测定血清磷脂脂肪酸谱,酶法测定血脂并进行比较。结果肥胖高血压组、非肥胖高血压组患者与健康人的血清磷脂脂肪酸谱有显著差异:主要表现为总SFA、总n-6PUFA显著高于对照组,总MUFA、总n-3PUFA、n-3/n-6PUFA显著低于对照组。肥胖高血压组总n-3PUFA显著低于非肥胖高血压组。肥胖高血压组、非肥胖高血压组与健康对照组相比血脂的变化为:TC、TG、LDL-C均显著上升,而HDL-C显著下降,肥胖高血压组与非肥胖高血压组间差别无统计学意义。相关性分析显示TC与C14:0呈显著正相关,与C18:1和C20:4n-6呈显著负相关;TG与C20:4n-6和n-3/n-6PUFA呈显著负相关;LDL-C与C18:0显著正相关,与C20:5n-3和n-3PUFA呈显著负相关;HDL-C与C16:1、C18:2n-6和C22:6n-3呈显著正相关,与C16:0呈显著负相关。结论高血压患者中总SFA和总n-6PUFA高于健康对照组,而总MUFA、总n-3PUFA和n-3/n-6PUFA低于健康对照组。肥胖高血压患者总n-3PUFA低于非肥胖高血压患者。SFA与高血压患者的TC、LDL-C升高有关,而MUFA和PUFA对高血压患者的血脂紊乱具有保护性作用。

【Abstract】 Objective To investigate serum phospholipid fatty acids profile in obesity-related hypertension patients, non-obesity hypertension patients and healthy subjects. Methods Serum specimens were collected from 41 obesity-related hypertension patients, 44 non-obesity hypertension patients and 53 healthy subjects. Serum phospholipid fatty acid was analyzed with capillary gas chromatography and serum lipids were measured by enzymatic assay. Results The composition of total SFA and total n-6PUFA were significantly higher in obesity-related hypertension group and non-obesity hypertension group than in control group, while total MUFA, n-3 PUFA and ratio of n-3/n-6PUFA were significantly lower than those in control group. Total n-3PUFA was significantly lower in obesity-related hypertension group than in non-obesity hypertension group. Compared with healthy controls serum lipid index TC, TG, and LDL-C in obesity-related hypertension group and non-obesity hypertension group were significantly higher than in healthy controls, while HDL-C was significantly lower than that in healthy controls. Bivariate correlation analysis showed that TC was positively correlated with C14:0, and negatively correlated with C18:1 and C20:4n-6; TG was significantly negatively correlated with C20:4n-6 and n-3/n-6PUFA; LDL-C was positively correlated with C18:0 and negatively correlated with C20:5n-3 and n-3PUFA; HDL-C was positively correlated with C16:1, C18:2n-6 and C22:6n-3 and negatively correlated with C16:0. Conclusion The present study suggests that the dietary SFA intake may contribute to blood lipid metabolism system, MUFA and PUFA intake may have a protective effect.
