基质金属蛋白酶表达和活性改变同心房颤动患者心房纤维化的关系

【摘要】 目的研究心房颤动(房颤)患者心房肌纤维化程度同基质金属蛋白酶(MMPs)表达和活性的关系,探讨MMPs在房颤患者心房重构中的作用。方法心外科手术病人53例,其中房颤患者27例,窦性心律患者26例,采用MASSON染色检测病理切片中胶原容积比例,逆转录-聚合酶链反应技术检测MMP-1、MMP-2、MMP-9的表达变化,通过明胶酶谱法观察MMP-2、MMP-9的活性改变。结果同窦性心律组比较,房颤患者的胶原容积比例增高,MMP-2mRNA水平和蛋白活性无明显变化,MMP-1、MMP-9mRNA升高,MMP-9的活性增强,差异有统计学意义(P<0.05)。结论心房纤维化在房颤的发生和维持中发挥重要作用,金属蛋白酶通过增强心房纤维化的程度而促进房颤的进展,参与房颤的心房结构重构,加重心房功能的衰竭。

【Abstract】 Objective To investigate the influence of atria] fibrillation(AF) on right atrial extracellular matrix and the relationship between AF and matrix metalloproteinases ( MMPs ). Methods Atrial tissues isolated from 26 patients with sinus rhythm and from 27 patients with chronic AF were processed for reverse transcription polymerase chain reaction ( RT-PCR) and zymography for detection of MMPs. Results Masson stain revealed increased collagen volume fraction( CVF) in AF group. RT-PCR revealed MMP-1、MMP-9 mR-NA contents increased in AF group and zymography analysis revealed upregulation of MMP-9 in right atrium in AF group. MMP-2 transcript and protein levels did not differ between two groups. Conclusion Atrial fibrosis increases in AF group and the change of MMP-1 and MMP-9 is associated with the perpetuation of AF and development of atrial structure remodeling.
