【摘要】 目的评价微创内固定手术治疗齿状突骨折合并寰枢椎脱位的临床疗效。方法18例齿状突骨折合并寰枢椎脱位患者,Ⅱ型骨折13例,Ⅲ型骨折5例,均伴寰枢关节前脱位。15例行经皮齿状突螺钉内固定,3例行经皮前路C1,2关节突螺钉内固定。结果寰枢关节及骨折复位满意,动力位X线片显示无寰枢椎不稳。16例达到齿状突骨性愈合,1例寰枢关节融合,另1例不愈合。结论微创内固定手术治疗齿状突骨折合并寰枢椎脱位,具有手术操作简单、创伤小、恢复快、疗效满意等特点,但应严格掌握手术适应证。更多还原

【Abstract】 Objective To evaluate the clinical outcomes of minimally invasive internal fixation in treatment of odontoid process fracture combined with atlantoaxial dislocation.Methods Eighteen cases of odontoid process fracture combined with atlantoaxial dislocation were analyzed retrospectively from March 2001 to June 2004.According to Anderson’s classification,there were 13 cases with type Ⅱ fractures and five with type Ⅲ,both combined with dislocations.Of all,15 cases were treated with percutaneous anterior odontoid screw fixation(PAOSF)and the other three with percutaneous anterior transarticular screw fixation(PATSF)under fluoroscope guidance.Results The average follow up was 12.6 months.Dynamic radiographies showed stability of atlantoaixal joint.Bony fusion was found in 16 cases,atlantoaxial joint in one and nonunion in one. Conclusion For odontoid fracture combined with atlantoaxial dislocation,minimally invasive internal fixation is characterized by simpleness,effectiveness,minor trauma and fast recovery.But the surgery indications should be controlled strictly.更多还原
