OSAS患者颅面上气道形态学与睡眠呼吸功能的相关性研究

【摘要】 目的研究OSAS患者的颅面上气道形态学与睡眠呼吸功能的相关性,探讨OSAS患者的与病情轻重有关的结构性指标。方法86名男性AH I≥15的OSAS患者平均为29±14.65,拍摄头颅侧位片,将颅面上气道33项指标与代表睡眠呼吸功能的AH I,AI,H I,LSaO2(夜间最低血氧饱和度)进行相关分析及多元线性逐步回归。结果以AH I,H I,LSaO2为因变量的逐步回归分析得到回归了方程,以AI为因变量未引出回归方程。结论上下中切牙越唇倾,下颌体长度越短,呼吸紊乱越严重。软腭后间隙越小,鼻咽径越小,舌骨位置越靠前,睡眠呼吸紊乱越严重。

【Abstract】 Objective To investigate the configuration index related to the degree of the state of an OSAS,bivariate correlation analysis was performed between the craniofacial structures,upper airway and the sleeping-respiratory disturbance of the OSAS.Methods The craniofacial skeletal and upper airway morphology of 86 male adult patients with AHI≥15 was measured by cephalometry and bivariate correlation and Multiple regression analysis was performed between the indexes of craniofacial structures,upper airway and AHI,AI,HI,LSaO2 which represent the degree of sleeping-respiratory disturbance of the OSAS.Results Multiple regression analysis showed that the equation were obtained from AHI,HI,LSaO2.Conclusion The variables of craniofacial structure and upper airway correlated with respiratory disturbance were: mandibular body length,the angle between upper and lower central incisor,back gap of the soft palate,the diameter of the nasopharynx,the sagittal position of hyoid.
