组织多普勒成像技术与冠状动脉造影对照评价冠心病心肌缺血的研究

【摘要】 目的:采用组织多普勒成像技术(TDI)定量测定冠心病心肌缺血患者的左心室各节段室壁运动速度,以评价左心室局部心肌的功能。方法:随机入选因胸痛住院并行冠状动脉(冠脉)造影的患者81例,通过冠脉造影确定冠心病组患者45例(冠脉狭窄≥50%),其余为对照组患者36例(冠脉狭窄<50%)。所有患者在行冠脉造影检查前均行普通超声心动图检查和左心室各室壁节段心肌组织多普勒成像技术(TDI)检查,分别比较结果。结果:冠心病组各节段室壁的收缩速度(Vs)较对照组无明显降低,舒张晚期运动峰速度(Va)两组比较无明显差异, 而舒张早期运动峰速度(Ve)及Ve与Va的比值均显著低于对照组相应节段。以冠脉狭窄≥70%为标准,按病变血管的种类分别与对照组相应节段心肌进行比较,不同部位严重狭窄的冠心病患者Vs、Ve及Ve/Va与对照组比较均有显著性差异(P<0.05)。结论:①冠心病组左心室局部心肌舒张功能异常的出现早于局部心肌收缩功能异常,TDI直观、定量显示左心室早期局部心肌舒张功能异常。②TDI亦能测定左心室局部心肌中严重缺血的心肌节段收缩功能异常。

【Abstract】 Objective: To evaluate the clinical value of tissue Doppler imaging(TDI) in quantitative detection of regional of left ventricular systolic and diastolic function in patients with coronary artery disease. Methods: Random examination was performed in 81 hospitalized patients by angiography. Forty-five were diagnosed as having coronary artery disease (CAD) and the other 36 were used as controls. Myocardial motion velocity was measured directly with pulsed-wave tissue Doppler imaging ( TDI). Results: Between the groups, there were significant differences in peak early diastolic velocity ( Ve) , peak late diastolic velocity (Va) ,and ratio of peak early/late diastolic velocity,except systolic velocity ( Vs). But in the serious stenosis groups, there were also significant differences in systolic velocity (Vs). Conclusions:① The patients with CAD showed earlier abnormality of diastolic function than did systolic function. TDI is valuable in the early diagnosis of CAD. ②TDI can detect regional myocardial systolic velocity especially in ischemic myocardium
