第3代头孢耐药的阴沟肠杆菌AmpC酶及ESBLs的表型测定

【摘要】 目的检测3代头孢耐药的阴沟肠杆菌,了解阴沟肠杆菌对3代头孢耐药的机制,以利于耐药性的控制与指导临床用药。方法收集2002年1月~2004年6月浙江省人民医院,来自各种标本对3代头孢耐药的阴沟肠杆菌165株,用三维试验检测ESBLs与AmpC酶。结果165株阴沟肠杆菌中,产ESBLs 120株,占72.7%,产AmpC酶有40株,占24.2%,同时产2种酶的有1株,占0.6%,2种酶均不产的有4株,占2.5%。体外抗生素敏感试验显示,亚胺培南、头孢替坦、头孢吡肟、环丙沙星的耐药率分别为1.2%、66.0%、92.1%和92.1%,氨苄西林、头孢唑啉的耐药率达到100%。结论该院对3代头孢耐药的阴沟肠杆菌主要产ESBLs,药物敏感试验表明亚胺培南敏感率最高(97.6%),头孢替坦次之(27.3%),而对其它抗菌药物敏感率较低。

【Abstract】 Objective To detect the ESBLs and AmpC β-latamases of third-generation cephalosporins-resistant E.cloacae,in order to investigate the resistant-mechanism of the third-generation cephalosporins with E.cloacae,in our(area),to help using durg of clinic.Methods To accumulate 165 strains of the third-generation cephalosporins-resistant E.cloacae in our hospital from January 2002 to July 2004.Detceted the ESBLs and AmpC β-latamases by three-dimensimal test.Results In 165 strains of E.cloacae,there were 120 strains producing ESBLs(72.7%),40 strains producing AmpC β-latamases(24.2%),and 1 strains producing both Ampc and ESBLs β-latamases(0.6%),4 strains producing neither Ampc or ESBLs B-latamases(2.5%),respectively.Antibiotics susceptibility test shows that the resistant rate of imipenem,cefotetan,cefepime,ciprofloxacin was 1.2%,66.1%,92.1%,92.1%,respectively.The resistant rate of ampicillin,cefazolin was 100%.Conclusion The most of the third-generation cephalosporins-resistant E.cloacae produce ESBLs in our hospital susceptibility rate to imipenem was higest(97.6%),cefotetan was the twith,the others were Very low.
