【摘要】 目的评价纤维支气管镜对大咯血患者手术止血定位的价值。方法11例因大咯血而内科治疗无效的患者,6例患者利用纤维支气管镜经气管插管或开胸后气管侧切口入支气管,判断出血部位后行肺段或肺叶切除术止血;5例患者通过肺部多层螺旋CT和临床体征判断出血部位,再行肺段或肺叶切除术止血。结果经纤维支气管镜定位后的6例大咯血患者,术后未出现大咯血;经肺部多层螺旋CT和临床体征定位的5例大咯血患者,有2例因再次大咯血二次手术,1例因再次大咯血死亡。结论纤维支气管镜可以准确判定大咯血所在肺段或肺叶的位置,对确定手术治疗方案有很大辅助作用。更多还原

【Abstract】 【Objective】 To evaluate the value of fibrobronchoscope in locating for surgical hemostasis of massive hemoptysic patients.【Methods】 Eleven patients who had been suffered from massive hemoptysis were collected.In order to judge the hemorrhagic sites,6 patients were given tracheal intubation via fibrobronchoscope or thoracotomy and lateral incision of trachea to enter the trachea;while the rest(5 patients) were based on lung CT and clinical sign.Then all of them were given segmentectomy or lobectomy.【Results】 The 6 patients who were locating via fibrobronchoscope did not relapse,while among the 5 patients locating via lung CT and clinical signs,1 patient died from another massive hemoptysis and 2 patients were given second operation because of second hemoptysis.【Conclusions】 Fibrobronchoscope can confirm the location of the massive hemoptysis accurately.It plays an important auxiliary role in ascertaining the scheme of surgical therapy.更多还原
