【摘要】 目的探讨膝关节镜下关节清理修复术治疗大骨节病膝关节炎的方法及疗效。方法31例大骨节病患者,按郭巨灵的临床分期:Ⅱ期2例,Ⅲ期20例,Ⅳ期9例。关节镜下摘除游离体;修整破损的半月板、剥脱的软骨;切削增生的滑膜;打磨股骨髁间窝、内外髁及胫骨平台的骨赘;在软骨破损骨裸露处钻孔。结果29例随访2年,主观指标:患者自我评价,显效28例,有效1例,总有效率100%;客观指标:疼痛指数;关节活动度;行走范围;30°、60°单腿站立。除关节活动度外其余客观指标均有明显改善。结论膝关节镜下关节清理修复术对治疗大骨节病膝关节炎具有良好的疗效。更多还原

【Abstract】 【Objective】 To observe the result of arthroscopic knee clearing and reconstructing operation for treating Osteo-arthritis of the knee in Kaschin-Beck disease.【Methods】 31cases of Kaschin-Beck disease,according to classification method of GUOJuling: type Ⅱ 2 cases,type Ⅲ 20 cases,type Ⅳ 9 cases.Under arthroscope,including clearing away dissociative object,hyperplastic synovium and bone;repairing meniscus and peeled cartilage,boring at the bare femoral and tibial condyle.【Result】 Subjective index was determined by patient’s evaluation of oneself: distinct effect 28 cases,effectivity 1 case,total effectivity is 100%.Objective indexs include pain index,active degree of joint,walk distance,stand on one leg at bending knee 300 and 600.Call back two years,except active dagree of joint,the rest index were improved to various degree.【Conclusion】 To treat Osteo-arthritis of the knee in Kaschin-Beck disease,arthroscopic knee clearing and reconstructing operation is very effective.更多还原
