靛氰绿-光动力学疗法对离体猪食管的作用
Effect of Photodynamic Therapy with Diode Laser and Indocyanine Green on Pig Esophagus in vitro
【摘要】 目的探讨靛氰绿(ICG)光动力学作用中激光照射猪食管的最佳功率和照射时间以及此等参数的激光对猪食管壁的损伤程度,为临床应用光动力学疗法治疗食管静脉曲张提供实验依据。方法采用不同功率的半导体激光照射离体猪食管标本,通过黏膜变性发白的功率时间关系,确定激光照射最佳功率;再采用该功率,对猪食管黏膜下分别注射靛氰绿溶液(实验组)和生理盐水(对照组)的猪食管离体标本进行照射,肉眼观察2组激光照射后猪食管壁的变化。结果激光照射最佳功率10W。采用功率为10W激光照射后,实验组猪食管黏膜激光照射时黏膜变性快,变性范围大,且深部组织不受损伤。对照组猪食管黏膜变性慢,变性面积小,且深部组织有损伤变性。结论激光照射离体猪食管功率10W较合适,黏膜下注射光敏剂靛氰绿可以加强治疗热效应,且对深部组织具有保护作用。
【Abstract】 Objective To explore the optimum power of diode laser for irradiating pig esophagus in vitro and investigate the mechanism of indocyanine green (ICG). Methods Esopagus specimens in vitro were irradiated by various power diode laser, and made power-time curve when esophageal mucous surface changed into whitish. According the curve we speculated the most optimum power of diode laser for irradiating fresh esophageal specimens. Then we used the power to irradiate two groups esophageal specimens respectively. In experimental group, ICG solution (1mg/ml) was injected into submucous space of esophagus specimens; in control group, normal saline was injected at the same volume. We investigated by nude eyes the changes in pig esophageal wall after laser irradiation. Results According to the curve we speculated that 10W was the most optimum power of diode laser for irradiating fresh esophageal specimens. We found that the experimental group was quickly denatured and had bigger denatured area than the control group when the same power laser irradiated on these esophageal mucous surfaces. After irradiating the muscular lamina of the experimental group was not injured, and the muscular lamina of the control one was whitish partly. Conclusions The most optimum power of diode laser for irradiating fresh esophageal specimens is 10 W. It can enhance thermal effect after injection of ICG solution into submucous space and protect deeper tissue to avoid being injured after laser irradiation on esophageal mucous surface.
