肾移植犬的急性排斥反应和电解质代谢
Acute Rejection and Electrolyte Unbalance after Kidney Transplantation in Beagle Dogs
【摘要】 目的观察肾移植犬的急性排斥反应与血液电解质代谢的变化。方法选体重为(12±2)kg的健康Beagle犬,进行同种异体肾移植手术,伪手术组只摘除右肾,保留左肾,移植组移植左肾,摘除右肾,定时进行血液生化指标监测,并记录肾移植犬的死亡数和存活时间。结果肾移植犬手术后3 d出现食欲下降、尿量减少,血清肌酐(Scr)和尿素氮(BUN)急剧上升,内生肌酐清除率(Ccr)水平急剧下降,K+浓度急剧上升、Ca2+浓度明显下降、Na+浓度呈下降趋势,均于6~10 d时死于急性排斥肾功能失调。结论肾移植犬手术后由于急性排斥反应,出现血肌酐清除率(Ccr)下降、血肌酐(Scr)和尿素氮(BUN)升高,同时并发低钠、高钾和低钙等电解质代谢的紊乱,而造成肾移植犬的肾功能衰竭。
【Abstract】 Objective To observe the acute rejection and changes of blood electrolytes after kidney transplantation in Beagle dogs.Methods Kidney allotransplantation was performed on 12 normal Beagle dogs weighing 12±2 kg.In the sham-operation group(n=6) only the right kidney was extirpated.In the operation group(n=6) extirpation of the right kidney and allotransplantation of the left kidney were performed.Several blood biochemical indices were measured before and after transplantation.The animal symptoms and survival were carefully observed and recorded.Results Appetite and urinary excretion of dogs with kidney transplantation were significantly decreased at three days after operation.Content of serum creatinine(Scr) and blood urea nitrogen(BUN) increased dramatically,endogenous creatinine clearance rate(Ccr) decreased badly,content of K+ increased drastically,content of Ca2+ reduced seriously,and the content of Na+ showed a degressive trend.All the 6 dogs of transplantation group died of renal failure caused by acute rejection from the 6~10 days after operation.Conclusion Acute rejection occurred in Beagle dogs after kidney transplantation,the content of Scr and BUN increased significantly,severe metabolic electrolyte unbalance such as elevation of K+ and decrease of Ca2+ and Na+ occurred.All those changes lead to acute renal failure.
