超声测量胎儿局部体积以估计胎儿体质量的方法学研究
Study on Ultrasound Methodology of Measuring Fetal Local Volumes Estimating Fetal Body Masses
【摘要】 目的通过超声测量胎儿身体局部体积求出能适用于低体质量儿、正常儿及巨大儿且较为准确的估计胎儿体质量(EFBM)公式。方法应用B超测量884例胎儿的头部体积(HeV)、腹部体积(AbV)及大腿部体积(ThV)并与新生儿出生体质量(NBM)进行相关分析,用多元回归的方法拟合估计胎儿体质量(EFBM)的回归方程。应用此方程对884例胎儿体质量进行前瞻性验证。结果经相关分析表明,应用胎儿的HeV、AbV、ThV综合建立的回归方程与NBM的相关系数为0.951,P<0.001。用此方程前瞻性EFBM884例,结果显示应用体积法EFBM的绝对误差在200g和300g以内分别占75%和89.6%;预测低体质量儿的符合率为80.9%、巨大儿的符合率为85.1%、对正常儿的预测符合率达93.7%。结论应用超声对胎儿身体部分体积测量方法得出的多元回归方程,准确性相对参数回归法更高、适用范围更广。
【Abstract】 Objective To seek a more accurate equation via ultrasound measuring the local volumes for estimating the fetal body masses(EFBM) in under-,normal- and macrosomia masses.Methods 1)The relationship between ultrasound measurements of fetal head volume (HeV),abdominal volume (AbV),thigh volume (ThV) and neonatal birth weight in 884 cases was studied.The correlation coefficients were analysed and a formula of multiple regression was established.2) The 884 fetuses were verified with the multiple regression equation respectively.Results The HeV,AbV and ThV correlated significantly to the neonatal birth mass with the coefficient of r=0.951,P<0.001.The stepwise regression analysis showed that the fetal mass was correctly predicted using the multiple regression formula with HeV ,AbV and ThV.The predictive study in 884 cases showed the absolute error at <200g and at <300g in 75 % and 89.6 % cases respectively;showed predictive values of the under mass fetus,macrsomia and normal mass fetus were 80.9%,85.1%and 93.7 % respectively.Conclusions The ultrasond volume measurement of the fetal HeV,AbV and ThV in EFBW derived from multiple linear regressive equation may reflect the various factors of the FBM.The formula accuracy rate is higher especially No 2 formula and the indication range will be relatively wider in clinical.
