脉冲多普勒组织成像评价急性心肌梗死对右室壁运动及左右心室间运动协调性的影响

【摘要】 目的采用脉冲多普勒组织成像技术(PW-DTI)评价急性心肌梗死(AMI)右室壁运动和左右室运动协调性的改变。方法首次AMI患者43例(前壁组23例,下壁组20例),于发病1周时行PW-DTI检测。测定三尖瓣外环Vs、Ve、Va;主、肺动脉电机械时间Q-Ao,Q-pul及IMD;二、三尖瓣外环电机械时间mQ-so,tQ-so及ΔQ-so。对照组19例行同样检测。结果前壁、下壁组三尖瓣外环Vs较对照组降低(P分别<0.05和0.01),下壁组三尖瓣外环Q-so较对照组与前壁组延长(P<0.05),下壁组ΔQ-so较对照组和前壁组减小(P<0.05和0.01)。结论前壁、下壁心肌梗死均可致右室壁运动幅度降低,且可致左右室运动协调性改变。

【Abstract】 Objective To quantitatively analyze the changes of right ventricular movement and interventricular synchrony parameters after acute myocardial infarction(AMI)using pulsed-wave Doppler tissue imaging(PW-DTI)Methods At one week after AMI,43 patients(anterior wall group,23 patients,inferior,20) were examinated with PW-DTI,Vs,Ve,Va were measured at the lateral tricuspid annulus.The time from the onset of QRS to the onset of aortic flow (Q-Ao) and pulmonary flow ( Q-pul) were measured.The difference between the two values were named IMD.The time from the onset of QRS to the onset of S wave at the lateral mitral annulus ( mQ-so) and tricuspid annulus (tQ-so) were measured and the difference between the two values were determined ΔQ-so.The same parameters were obtained in 19 contral subjects.Results At the lateral tricuspid annulus,the Vs of both anterior and inferior group were decreased significantly when compared with the control group(P<0.05 and <0.01 respectively),tQ-so was longer in the inferior group than in the control group and anterior group (P<0.05) and ΔQ-so in the inferior group was shorter than in the anterior group(P<0.01).Conclusions After AMI,the movement of RV wall were decreased in both the anterior and inferior groups and where the interventrucular dissynchrony was taken place.
