右心容量负荷对小儿右心长轴局部心肌应变和应变率的影响
Effects of Volume Overload on Right Ventricular Longitudinal Strain and Strain Rate in Children
【摘要】 目的应变(strain,ε)、应变率(strain rate,SR)显像是评价右心局部心肌功能的新方法,影响ε和SR的因素有很多,本文主要研究右心容量负荷过重对右心室长轴收缩期峰值ε及SR的影响。方法20例房间隔缺损(ASD)患者,与20例正常对照者,获取心尖四腔右室游离壁长轴方向基底段、中间段的心肌ε、SR曲线,测定其收缩期峰值ε和SR。结果右室游离壁中间段心肌ε、SR高于基底段,ASD组右室游离壁长轴方向各节段心肌收缩期峰值ε、SR均显著高于正常对照组相应节段的测值(P<0.01)。结论当右室容量负荷过重时,右室游离壁长轴方向心肌收缩期峰值ε、SR明显增高,提示ε和SR显像对局部心肌功能的评价受容量负荷的影响。
【Abstract】 Objective To assess the altered ventricular preload on right ventricular longitudinal strain and strain rate.Methods The right ventricular longitudinal strain(ε) and strain rate(SR) were measured in right ventricular free wall from apial four chamber views in 20 patients with atrial septal defect(ASD)and 20 healthy children respectively.Results In patients wih ASD),the strain and strain rate in both the mid and basal segments of right ventricular free wall strain and strain rate were significantly increased than those in the healthy children.Longitudinal systolic peak ε and SR in the mid part of the RV free wall were higher than the basal part both in ASD and healthy children.Conclusions The right ventricular longitudinal strain and strain rate is markedly influenced by preload.
