二维及M-型超声心动图技术评价艾滋病患者左心室收缩功能
Assessment of Left Ventricular Systolic Dysfunction in AIDS Patients with Two Dimensional and M-mode Echocardiography
【摘要】 目的应用二维及M-型超声心动图技术评价艾滋病患者左心室收缩功能的变化。方法把入选的129例艾滋病患者,按病程进展分为人类免疫缺陷病毒(H IV)感染者组57例和获得性免疫缺陷综合症(A IDS)患者组72例;另随机选取64例健康志愿者作为对照组。对3组研究对象的左室收缩运动进行二维及M-型超声心动图测量并作对比分析。结果HIV感染者和AIDS患者与对照组相比,左室射血分数(EF)、短轴缩短率(FS)、舒张期室间隔厚度(IVSTd)及其增厚率(ΔIVSTd%)、舒张期左室后壁厚度(LVPWTd)及其增厚率(ΔLVPWT%)均明显减低,左室舒张末期内径(LVIDd)、收缩末期内径(LVIDs)明显增大;AIDS患者与HIV感染者相比,EF、FS明显减低,LVIDd、LVIDs明显增大。结论艾滋病患者左室收缩功能发生了明显改变,随着病程进展,其受损程度逐渐加重。二维及M-型超声心动图技术在评估艾滋病患者左室收缩功能变化中具有重要应用价值。
【Abstract】 Objective To assess left ventricle(LV) systolic function in acquired immunodeficiency syndrome(AIDS) patients with two dimensional and M-mode echocardiography.Methods Totally 129 HIV patients were classified into two groups according to infection situations in patients.One group consisted of 57 asymptomatic HIV-seropositive patients and the other group consisted of 72 AIDS patients.64 HIV-seronegative volunteers selected for normal control group.LV systolic function of three groups were assessed with two dimensional and M-mode echocardiography and were comparatively analyzed.Results Compared to normal controls,HIV-seropositive patients and AIDS patients showed significantly decreased LV ejection fraction(EF),LV fractional shortening(FS),end-diastolic interventricular septum thickness(IVSTd) and its increased rate in end-systole(ΔIVSTD%),end-diastolic LV posterior wall thickness(LVPWTd)and its increased rate in end-systole(ΔLVPWT%)and significantly increased LV inner diameter in end-diastole(LVIDD) and in end-systole(LVIDS).Compared to HIV-seropositive patients,AIDS patients showed significantly decreased EF,FS and significantly increased LVIDD and LVIDS.(Conclusions LV) systolic function was significantly altered in HIV patients and was gradually deteriorated as advancing of pathogenesis.Two dimensional and M-mode echocardiography showed great clinic value in assessing LV systolic function in HIV patients.
