2005年贵州省第5次碘缺乏病监测结果分析
The fifth surveillance on iodine deficiency disorders in Guizhou Province in 2005: an outcome analysis
【摘要】 目的了解2005年贵州省碘缺乏病(IDD)防治现状,完善可持续发展防治策略,使贵州省尽快实现消除IDD阶段目标。方法根据全国第5次IDD监测方案要求进行,用PPS抽样法在全省抽出30个县。8~10岁儿童甲状腺检查采用触诊法和B超法,尿碘测定采用过硫酸铵消化—砷铈催化分光光度法,盐碘采用氧化还原滴定法(B/T 13025.7-1999),智力测定采用《中国联合型瑞文测验CRT-C2图册》进行,5年级学生及18~35岁育龄妇女健康教育采用问卷调查。结果8~10岁学生甲状腺肿大率触诊法4.42%,B超法3.80%;儿童尿碘中位数289.4μg/L,居民户家中盐碘中位数34.1 mg/kg;儿童智商(IQ)均值92.8,5年级学生健康教育问卷调查平均分68.23,18~35岁育龄妇女平均分73.87;历年监测结果表明,2005年居民合格碘盐食用率逐年提高,尿碘中位数趋近于WHO推荐的100~200μg/L的最适宜值,儿童甲状腺肿大率降低。结论贵州省人群碘营养状况不断得到改善和提高,碘缺乏病得到了有效的控制,今后要继续坚持可持续发展防治策略,深入开展健康教育工作。
【Abstract】 Objective To investigate the current status of iodine deficiency disorders (IDD) in 2005 to persistently perfect the prevention strategy aiming at eliminating IDD. Methods According to" The Fifth National IDD Surveillance Scheme", 30 counties screened by the method of proportional population sampling were surveyed. Colorimetric eerie-arsenic assay and vitriolic ammonium assimilation were used for testing urinary iodine, while oxidation-reduction eerie-arsenic assay for salt iodine (B/T 13025.7-1999). The size of thyroid gland was measured by palpation or B-ultrasound. Ravens test was used to test the intelligent quotient(IQ). Questionnaires were performed on health education status of students of the fifth grade and women aged 18~35 years old. Results The rate of goiter were detected in a rate of 4.42% by palpation, 3.8% by B ultrasound. The median of urinary iodine was 289.4μg/L, while the median of salt iodine was 34.1 mg/kg. The average IQ was 92.8 of the students aged 8~10. The average score of the healthy and educational questionnaires was 68.23 among the students in the fifth grade, and was 73.87 among women aged 18~35. The rate of qualified iodized salt consumption, as the survey showed, was increased gradually resulting a reduced rate of goiter and a optimal median of urinary iodine of 100~200μg/L, which was recommended by WHO. Conclusions For iodine nutrition keeps improved, IDD has been effectively controlled. Preventive strategy should be further developed with more attention paid on health education.
