【摘要】 目的探讨小切口手指引导复位顺行带锁髓内钉治疗狭部以远A型和B型股骨干骨折的手术方式及疗效。方法自2001年4月～2004年12月,对26例狭部以远A型和B型股骨干骨折取小切口手指引导复位顺行带锁髓内钉固定。结果手术时间60～120min,平均75min。骨折小切口处失血5～10ml。本组26例骨折均达或接近解剖对位。平均随访18.5个月,骨折临床愈合时间平均12周(10～16周),无骨延迟愈合及骨不愈合,无1例感染,无一主钉断裂,髋膝关节功能均达术前,无膝关节内外翻发生,远端锁钉退出2例。结论小切口手指引导复位顺行带锁髓内钉固定是治疗狭部以远A型和B型股骨干骨折的简易理想方法,具有投资小,操作简便,创伤小,骨折愈合快,关节功能恢复好等特点,值得推广应用。更多还原

【Abstract】 Objective To explore the operative manners and its effects of antegrade interlocking intramedullary nail in treating femoral shaft fractures of type A and B distal to the isthmian with finger reduction by mini-incision.Methods Twenty-six cases of femoral shaft fractures of type A and B distal to the isthmian were treated by finger leading reduction in mini-incision and antegrade interlocking intramedullary nail from April 2001 to December 2004.Results Operation time was 60～120 min,average 75 min.The bleeding of mini-incision was 5～10 ml.All of the patients were followed up for an average period of 18.5 months.The factures were all restored significantly.The average fracture clinical healing period was 12 weeks(10～16weeks),without delayed unions and nonunion,without infection,without broken nail,without any change in the function of hip and knee joint before operation,without occurred genu varum and genu valgum.the distal nail seceded in only 2 cases.Conclusion The antegrade interlocking intramedullary nail in treating femoral shaft fractures of type A and B distal to the isthmian with finger reduction in mini-incision is a easy effectual device,lies in small investment,mini invasion,easy operation,rapid healer of the fracture,good function of the joint,and it may be applied to clinic widely.更多还原
