食管癌和癌旁组织磷酸化酪氨酸蛋白和C-erbB-2的表达及意义

【摘要】 目的探讨食管癌变过程中细胞增生调控的分子学机制。方法利用免疫组织化学技术研究66例原发性食管癌和癌旁正常及各级病变组织中p-Tyr蛋白和C-erbB-2表达状况,进行检测分析。结果p-Tyr蛋白和C-erbB-2从正常食管上皮到基底细胞过度增生、不典型性增生和鳞癌,免疫阳性表达率分别为18%、46%、58%、61%和14%、60%、37%和89%;从高分化到中分化及低分化鳞癌,阳性率分别为82%、64%、44%和94%、100%和75%;食管癌组织中C-erbB-2免疫反应阳性病灶中p-Tyr阳性率为64%(21/33),C-erbB-2免疫反应阴性病灶中p-Tyr阴性率为50%(2/4),但p-Tyr和C-erbB-2相关性分析显示二者无相关性;从形态学正常的食管上皮到BCH、DYS、SCC,p-Tyr和C-erbB-2局灶型分布的百分率分别为46%、66%、36%、37%和50%、67%、48%和35%,弥漫型分布的百分率分别为44%、34%、64%、53%和50%、33%、52%和65%。结论p-Tyr蛋白和C-erbB-2表达变化发生于食管癌变早期阶段,二者可能是参与食管癌变发生过程的重要分子学事件;p-Tyr蛋白可能是食管癌预后的重要生物学指标之一,酪氨酸蛋白的磷酸化可能是食管上皮癌变多阶段演进过程中细胞增生调控相关的重要信号转导形式之一。

【Abstract】 Objective To further elucidate the molecular events involved in the esophageal carcinogenesis.Methods 66 specimen were obtained from normal esophageal epithelium tissues with basal cell hyperplasia(BCH),dysplasia(DYS) and squamous cell carcinoma(SCC).The expressions of phosphotyrosine protein(p-Tyr) and C-erbB-2 were determined by immunohistochemistry method in all specimen.Results From the normal esophageal epithelium→BCH→DYS→SCC,the positive immunostaining rates for p-Tyr and C-erbB-2 increased gradually,the positive rates were 18%,46%,58%,61% and 14%,60%,37%,89%,respectively;the significant difference was observed between normal epithelium and lesions with different degree of severity(P<0.05).From well-differentiated to moderate-differentiated and poorly-differentiated SCC,the positive immunostaining rates for p-Tyr and C-erbB-2 decreased gradually,the positive rates were 82%,64% and 44% and 94%,100%,75%,respectively.The positive rate for p-Tyr in lesions with C-erbB-2 positive immunostaining was 64%(21/33),the negative rate for p-Tyr in lesions with C-erbB-2 negative immunostaining was 50%(2/4).There was no correlation between p-Tyr and C-erbB-2.From normal esophageal epithelium to BCH→ DYS→SCC,the local pattern rates and the diffuse pattern rates of immunostaining for p-Tyr were 46%,66%,36%,37% and 44%,34%,64%,53%,respectively;the local pattern rates and the diffuse pattern rates of immunostaining for C-erbB-2 were 50%,67%,48%,35% and 50%,33%,52%,65%,resp ectively.Conclusion The alterations of p-Tyr and C-erbB-2 protein expression occur in the early stage of esophageal carcinogenesis,suggesting that C-erbB-2 and p-Tyr may be implicated in esophageal carcinogenesis.The positive immunostaining rates for p-Tyr decreased significantly from well-differentiated to poorly-differentiated SCC,which indicate that p-Tyr may be an important prognostic biomarker for EC and that phosphorylation of tyrosine protein may be an important pathway for signal transduction involved in regulation cell proliferation in esophageal multistage carcinogenesis.
