99mTc-P53和99mTc-N(NOEt)2在肺部包块显像方面的比较研究

【摘要】 目的研究99mTc-P53和99mTc-N(NOEt)2显像对肺癌的诊断及鉴别诊断方面的临床价值。方法用99mTc-P53和99mTc-N(NOEt)2分别对44例有病理诊断的肺部包块的病人进行SPECT显像。结果肺癌99mTc-N(NOEt)2的早晚期T/N比值差异有显著性。为判断良恶性的标准,99mTc-P53诊断的敏感性、特异性及准确性分别为91%,55%和81%,而99mTc-N(NOEt)2诊断的敏感性、特异性及准确性分别为88%,64%和81%。肺炎性假瘤对99mTc-P53和99mTc-N(NOEt)2都有比较高的摄取,二者比较无统计学意义。断层显像要优于平面显像。肺癌的大小不是决定肺99mTc-P53和99mTc-N(NOEt)2是否被浓聚的必然因素。在肺癌显像方面,99mTc-P53和99mTc-N(NOEt)2SPECT无显著性差异,二者与肺癌的不同TNM分期也无显著性差异。结论99mTc-P53和99mTc-N(NOEt)2对肺癌的显像有着良好的敏感性,但都相对缺乏特异性。

【Abstract】 Obiective To evaluate the clinical value of 99mTc-P53 and 99mTc-N(NOEt)2 imaging for the diagnosis of lung neoplasms.Methods The SPECT imagings were performed on 44 patients with pulmonary lesions.Results The senstitivity,specificity and overall accuracy of 99mTc-P53 imaging for the malignant leson was 91%,55%,81%,respectively.The senstitivity,specificity and overall accuracy of 99mTc-N(NOEt)2 imaging for the malignant lesion was 88%,64%,81%.There was no significant differance in different histological entities of the lung carcinoma.The probability of 99mTc-P53 and 99mTc-N(NOEt)2 uptake in the mass was not related to the mass size.Four of the nine benign lung lesions(4 cases of inflammatory pseudotumor) and 5 of the nine benign lung leisons(5 cases of inflammatory pseudotumor) showed uptake.But there was no significant differance between 99mTc-P53 and 99mTc-N(NOEt)2 imaging.Conclusions Both 99mTc-P53 and 99mTc-N(NOEt)2 are highly sensitive in detecting malignant lesions,but they have poor specificity.
