继发性肝脏淋巴瘤的CT诊断

【摘要】 目的:提高对继发性肝脏淋巴瘤的CT表现的认识。材料和方法:回顾性分析2001年12月至2004年8月共7例经病理证实的淋巴瘤,其中霍奇金淋巴瘤4例,非霍奇金淋巴瘤3例,所有病例在发病时或随访过程中肝脏出现占位性病变,治疗后肝脏病灶均有缩小或消失,以此推断淋巴瘤肝脏浸润。所有病例均经超声检查发现肝占位,进而螺旋CT平扫与增强扫描进一步明确。结果:在7例继发性肝淋巴瘤中,肝孤立性占位1例(14%),6例(86%)表现为多发占位。CT平扫均为低密度;增强后5例无明显强化,2例出现边缘强化。结论:尽管继发性肝脏淋巴瘤缺乏特征性CT表现,但结合临床资料,继发性肝脏淋巴瘤可以被正确诊断。

【Abstract】 To improve realization the CT characteristics in patients with of hepatic lymphoma. Materials and Methods:We reviewed seven patients in whom a diagnosis of lymphoma was finally made histologically during a 33-month period (Dec 2001 through Aug 2004) . In all patients, four of them were diagnosed Hodgkin’s disease(HD) and three of them were diagnosed with non-Hodgkin’s lymphoma(NHL). Liver lesions were observed at the onset of the disease or during the follow-up period. Because Some liver lesions disappeared in some cases and others reduced after treatment, thus, we concluded liver was infiltrated by lymphoma. Liver lesions were found in all patients by ultrasound and then underwent which were further verified by unenhanced and contrast-enhanced spiral CT scan.Results:In all seven cases,only one presented single liver lesion (14%),the other 6 cases presented multiple lesions (86%). CT showed hypoattenuating lesions with attenuated intensity in all cases.After administrating contrast-medium, there was no evident enhancement of lesion intensity in five cases and with rim enhancement was seen in two cases.Conclusion:Combining main CT characteristics and combining clinical information manifestations, secondary liver lymphoma can be identified correctly.
