HLA-DRB1基因分型与慢性乙型肝炎的相关性研究

【摘要】 目的探讨HLA-DRB1等位基因多态性与慢性乙型肝炎之间的关系。方法采用聚合酶链反应、反向特异性序列寡核苷酸探针(SSO)基因分型技术结合Luminex液态芯片技术对81例慢性乙型肝炎患者和90例正常人的HLA-DRB1等位基因进行分型比较。结果HLA-DRB1等位基因0901型在慢性乙型肝炎的等位基因频率明显高于对照组(33.33%、15.56%),两者相比差异有显著性(χ2=7.40,P<0.01,RR=2.71),而HLA-DRB1等位基因0403型在慢性乙型肝炎的等位基因频率明显低于对照组(16.05%、35.56%),两者相比差异有显著性(χ2=8.37,P<0.01,RR=0.37);其他型别的等位基因2组间没有明显差别。结论HLA-DRB1等位基因0901型与慢性乙型肝炎易感性密切相关,可能是慢性乙型肝炎的易感基因或连锁基因;HLA-DRB1等位基因0403型与慢性乙型肝炎抗性密切相关,可能是慢性乙型肝炎的抗性基因。宿主的HLA-DRB1是乙肝病毒感染转归的重要因素。

【Abstract】 Objective To investigate the association between chronic hepatitis B(CHB) and the polymorphism of HLA-DRB1.Methods The HLA-DRB1 alleles in 81 patients with CHB and 90 normal control subjects were analyzed by using the polymerase chain reaction∕sequence specific oligonucleotide probes(PCR SSOP) technique and the Luminex liquid CMOS chip.Results The frequency of HLA-DRB10901 allele in the CHB group was markedly higher than that in the normal control group(33.33% vs 15.56%,with significant correlation between them,χ~2=7.40,P<0.01,RR=2.71);the frequency of HLA-DRB1*0403 in the CHB group was lower than that in the normal control group(16.05% vs 35.56%,statistically significant,χ~2=8.37,P<0.01,RR=0.37).No significant differences among the other alleles.Conclusion HLA-DRB10901 is closely associated with the susceptibility to CHB,and may be the susceptible gene or linkage gene.HLA-DRB10403,in inverse proportion to the susceptibility to CHB,may be an oppositional gene.They two may regulate the outcome of the HBV infection in a victim.
