16排螺旋CT三维重建在寰枢椎损伤中的应用

【摘要】 目的:回顾性分析16排螺旋CT扫描及三维重建技术在寰枢椎损伤中的诊断价值。方法:33例寰枢椎损伤患者,均行16排螺旋CT扫描及后处理重建(包括SSD,MPR,VR),平扫层厚2.5mm或5.0mm,重建层厚0.625mm或1.25mm,其中22例患者行普通X线检查,10例行MR检查。结果:33例患者中,单纯横韧带损伤1例,寰椎骨折5例,寰枢关节脱位8例,枢椎骨折19例,多排螺旋CT及后处理技术可以清晰显示骨折的部位及分型,寰齿间隙的改变。平片漏诊率为22.7%(5/22),轴位图像漏诊率为6.1%(2/33),结合三维后处理漏诊率为0%,MR能清晰显示脊髓损伤情况。结论:多排螺旋CT三维重建技术是诊断寰枢椎损伤的首选方法,结合MR可以进一步了解脊髓损伤情况。

【Abstract】 Objective:To analysis the diagnostic value of 16-slice helical CT and three-dimensional reconstruction retrospectively in atlas and axis injury.Methods:33 patients with atlas and axis injury underwent spiral CT scan,then three-dimensional CT reconstruction(include surface shaded display,SSD;multiplanar reconstruction MPR;volume rendering,VR).Thick 2.5mm or 5.0mm,reconstruction,thick 0.625mm or 1.25mm,at the same time,22 patients underwent X-ray examination and 10 patients did MR.Results:Among 33 cases,only 1 case was simple ligamentum transversum injury,5 cases were atlas fracture,8 cases atlas-axis joint dislocation and 19 cases axis fracture.MSCT and three dimensional reconstruction can clear display the location and type of fracture,the change of atlas odontoid.The rate of misdiagnosis in X-ray plain film was 22.7%,in axial view of CT scan was 6.1%,no diagnosis was missed in three dimensional reconstruction,the MR can display the spinal cord injury clearly.Conclusion:Multi-slice helical CT three-dimensional reconstruction imaging is a ideal method in atlas and axis injury,and may get the message of spinal cord injury with MR.
