正常腰椎骨髓高场强MRI信号类型探讨

【摘要】 目的:探讨正常人体腰椎骨高场强MRI信号随年龄、性别变化的规律及类型。方法:随机筛选了470例正常腰椎的MRI影像,男235例,女235例。按10岁一年龄组,共分8组。以T1WI为重点进行回顾性观察,按信号表现归类,对信号类型与年龄性别的关系做统计学处理。结果:依据椎体内信号的不同表现将其分为5型(6亚型),Ⅰ型与年龄负相关(r=-0.781,P=0.00),随年龄增长而减少,86%年龄小于20岁,30岁以上罕见;Ⅱ型与年龄负相关(r=-0.661,P=0.00),随年龄增长而减少,多见于40岁以下。Ⅲ型与年龄正相关(r=0.804,P=0.00),随年龄增长而增多,其中Ⅲa型在20~30岁有个高峰期,Ⅲb见于40岁之后;Ⅳ型信号分布与年龄正相关(r=0.748,P=0.00),随年龄增长而增多,多见于30岁以上;Ⅴ型信号分布与年龄无相关性(r=-0.234,P=0.099),均见于各年龄组。不同性别与信号类型差异无显著性意义(P>0.05)。结论:腰椎骨髓的MRI信号随年龄而变化,骨髓转化以局灶、不均匀为主。高场强磁共振可以更清楚地、准确地反映骨髓转化的生理特点,了解骨髓变化规律对正确认识正常腰椎骨髓及病变的鉴别诊断有极其重要的意义。

【Abstract】 Objective:To investigate the signal patterns of the bone marrow of normal lumbar vertebra with regards to the age and sex.Methods:470 cases of MRI of the normal lumbar vertebra(male 235,female 235),randomly selected for a retrospective study and divided into eight age groups to assess the variation of the signal.All vertebrae were mainly evaluated and classified according to the signal characteristic in T1-weighted images.The results were correlated with the age and sex statistically.Results:The bone marrow patterns were divided into 5 pattern(6 sub-patterns).Pattern Ⅰ: The pattern were negatively correlated with ages(r=-0.781,P=0.00),the frequency of pattern Ⅰ decreased with age(86% of those younger than 20 years and dropping to nearly zero of those older than 30 years).Pattern Ⅱ: The patterns were negatively correlated with ages (r=-0.661,P=0.00),the frequency of pattern Ⅱ decreased with age,were often found in patients younger than 40 years.Pattern Ⅲ: The patterns were positive correlated with ages(r=0.804,P=0.00),the frequency of pattern Ⅲ increased with age.Pattern Ⅲa had the top time from 20 yeass to 30 years, pattern Ⅲb were often found in patients older than 40 years.Pattern Ⅳ: The patterns were positive correlated with ages(r=0.748,P=0.00),the frequency of pattern Ⅳ increased with age,were often found in patients older than 30 years.Pattern Ⅴ: The patterns were not correlated with ages(r=-0.234,P=0.099),could be found in any age groups.Statistical analysis demonstrated that the variation of bone marrow patterns was not related to sex(P>0.05).Conclusion:MRI signal patterns of normal lumbar vertebra change with ages.The normal MRI pattern of marrow conversion of lumbar vertebra is mainly in focal spotted pattern.High magnetic-field MRI is sensitive in detecting signal change between fatty and haematopoietic marrow.Knowledge of the different patterns may serve as a practical aid in the interpretation of MRI between normal and abnormal lumbar vertebra.
