【摘要】 目的探讨室间隔缺损合并主动脉瓣关闭不全的外科治疗时机和手术方法。方法自1987年9月至2005年12月期间,我们共施行外科手术治疗室间隔缺损(VSD)合并主动脉瓣关闭不全(AI)87例,其中属动脉干下型室间隔缺损者60例、膜周型者27例。手术均采用全身中度低温体外循环。4例直接修补室间隔缺损,余均采取补片修补术。对主动脉瓣病变,施行瓣膜成形术31例,10例无法做者则施行主动脉瓣置换手术,其余46例单纯行室间隔补片修补术,对主动脉瓣未做特殊处理。结果手术效果满意。术后随访6个月至10年,全组病例无死亡,心功能Ⅰ级55例,Ⅱ级32例。结论对室间隔缺损合并主动脉瓣关闭不全的患者进行同期外科治疗可以取得良好的治疗效果。更多还原

【Abstract】 Objective To explore the surgical method and optimal time to correct ventricular septal defect (VSD)associated with aortic valve insufficiency(AI). Methods Between September 1987 and August 2005, 87 VSD associated with AI patients were treated. 60 patients had subarterial VSD, 27 cases had membranous defect. Moderate hypothermia circulatory arrest were applied in all cases. Direct closure of the VSD was undertaken in 4 cases and other cases repaired with patches. Aortic valvuloplasty in 31 cases, and 10 patients underwent aortic valve replacement, other 46 cases had VSD repaired with patches. Results The results of surgical treatment is satisfactory. In the 6 months to 10 years follow up, all patients survived, 55 cases were in cardic function NYHA class Ⅰand 32 in NYHA classⅡ. Conclusion Concomitant surgical treatment ventricular septal defect associated with aortic valve insufficiency associated with ideal results.更多还原
