【摘要】 目的探讨适宜近视激光手术人群的角膜直径在性别之间、眼别之间的差别以及与不同近视屈光度的相关性。方法对来本院屈光手术中心就诊的220例(440眼)近视患者(17~41岁,男118例,女102例)采用美国Lasersight公司的AstraMax角膜地形图进行角膜前表面摄像,测量3次并获取角膜水平直径(白到白)的平均值,并对其角膜直径进行相关的统计学分析。结果本组病例中角膜直径平均值为(12.00±0.40)mm,男性角膜直径为(12.12±0.36)mm;女性为(11.86±0.40)mm;右眼角膜直径为(12.02±0.40)mm,左眼为(11.99±0·41)mm。对低度、中度和高度近视者的角膜直径的平均值分别为12.15mm、12.00mm、11.91mm。男女性别之间角膜直径存在着统计学差别;眼别之间没有统计学意义;3组近视屈光度和角膜直径之间方差分析结果显示高度近视角膜直径最小,低度近视角膜直径最大,三者间存在统计学差别;近视屈光度和角膜直径之间存在着统计学负相关性。结论应用AstraMax角膜地形图系统测量角膜直径可行性良好,该研究对于近视激光手术的手术设计有着重要的意义,另外该结果可为大小角膜的准确定义提供一定的参考。更多还原

【Abstract】 Objective To determine whether AstraMax topography system detects the difference in corneal diameter between the genders,between right and left eyes,and the correlation between diopter and corneal diameter.Methods Corneal diameters were measured with AstraMax topography system in 220 right eyes and 220 left eyes of 220 healthy subjects aged between 17 to 41 years.There were 102 female subjects and 118 male subjects.Each measurement was repeated three times.Results The average corneal diameter was (12.00±0.40) mm.The average corneal diameter was (12.12±0.36) mm in males compared with (11.86±0.40) mm in females;(12.02±0.40) mm in right eyes and (11.99±0.41) mm in left eyes.Corneal diameters classified with refractive error were 12.15 mm,12.00 mm and 11.91 mm respectively for low,moderate and high myopia.Differences in gender were significant in the t test for independent samples(P=0.000).There were no statistically significant differences between right and left eyes in the t test for dependent samples(P=0.503).There were statistically significant differences for corneal diameters classified with refractive error.The corneal diameter was negatively correlated with the refractive error.Corneal diameters decreased with refractive error diopters.The higher the refractive error,the less the corneal diameter.Conclusion AstraMax topography system provide us a precise information of corneal diameter,the corneal diameter was significantly greater in males than in females,corneal diameters decreased with refractive error diopters.It is important to choose the operative style before the refractive surgery.With the obtained normal values,more precise determination of microcornea and macrocornea will be possible in the future.更多还原
