【摘要】 目的总结肾血管平滑肌脂肪瘤(renalangiomyolipoma,RAML)出血的诊治经验。方法20例RAML出血患者,14例肿瘤直径>4cm出血难自止者经急诊手术治疗,其中行肾切除8例,肾部分切除和肿瘤剜除术各3例;肿瘤直径<4cm的6例先行抗休克治疗后行肾切除术、肿瘤剜除术各3例。结果术后病理检查均为RAML破裂出血。随访6～36月肿瘤无复发,行保肾手术(肾部分切除、肿瘤剜除术)的9例患肾功能正常。结论B超及CT对RAML出血的诊断具有重要意义,治疗原则常根据肿瘤大小及症状而定。更多还原

【Abstract】 Objective To summarize the diagnosis and treatment of rupture and hemorrhage of renal angiomyolipoma(RAML). Method 20 cases of RAML complicated with rupture and hemorrhage were reviewed. 14 cases with tumor larger than 4 cm in diameter had uncontrolled bleeding who underwent emergency operation, including 8 cases of nephrectomy, 3 cases of partial partial nephrectomy and 3 cases of tumor wedging out.6 cases with tumor smaller than 4 cm in diameter underwent conservative treatment followed by nephrectomy and tumor wedging out in 3 cases each. Result Postoperative pathological examination revealed the diagnosis of rupture and hemorrhage of RAML. No recurrence with 6 to 36 months follow-up, 9 cases with kidney preserved showed normal renal function. Conclusion Ultrasound and CT scan are sensitive to diagnose the RAML, treatment is varied with the size of tumor and the symptom.更多还原
