【摘要】 目的总结采用游离组织瓣移植行口腔颌面部缺损组织重建的方法及经验。方法对2001年2月～2004年10月采用游离组织瓣移植行口腔颌面部组织缺损重建的82例患者作回顾性研究,分析采用组织瓣的类型、修复部位、受区血管、组织瓣成活情况及术后并发症的发生情况。结果前臂皮瓣、腓骨瓣和腹直肌瓣为我科目前应用的行口腔颌面部重建的游离组织瓣,其中前臂皮瓣52例,腓骨皮瓣22例,腹直肌瓣8例；舌部缺损49例,颊部缺损5例,下颌缺损22例,腭部缺损6例；游离组织瓣移植的成功率为97．6%(80/82),术后血栓的发生率为4．9%(4/82),抢救成功率为50%(2/4)；受区并发症发生率为11%(9/82)。结论游离组织瓣移植安全可靠,且优于传统的带蒂组织瓣移植,是口腔颌面部重建的理想选择。更多还原

【Abstract】 Objective To evaluate the clinical application of vascularized free flaps in reconstructing oral and maxillofacial defects.Methods We retrospectively analysed 82 cases with oral and maxillofacial defects which had been reconstructed with vascularized free flaps from February 2001 to October 2004.Data concerning the flap types,the defect description and sites,the survived rates,recipient vessels and complications were analysed.Results The free radial forearm flap was most commonly used,followed by free fibula flap and rectus abdominis flap.The overall success rate of flaps was 97.6% (80/82).The overall complication rate was 15.9% (13/82).The overall thrombosis rate was 4.9% (4/ 82)and the flap salvage rate was 50% (2/4).Conclusion Free flap transfer in oral and maxillofacial region is safe and reliable.It is superior to the conventional pedicled flap technique by reason of less complication and high successful rate.更多还原
