血糖控制对初发2型糖尿病患者C-反应蛋白及白细胞介素6水平的影响
Effects of glucose control on the levels of C reactive protein and interleukin-6 in patients with newly onset type 2 diabetics
【摘要】 目的:观察初发2型糖尿病患者血清C-反应蛋白及白细胞介素6的水平以及降糖治疗对C-反应蛋白及白细胞介素6的影响。方法:①选择2003-02/06江苏省人民医院内分泌科初发2型糖尿病患者48例做为糖尿病组,符合美国糖尿病协会标准。男22例,女26例,年龄(50±7)岁,体质量指数(25.64±2.92)kg/m2,均对治疗方案及检测指标知情同意。给予口服降糖药物复方二甲双胍治疗(0.25~0.5)g,3次/d,治疗3个月控制血糖,采用光学发光法及酶联免疫反应方法分别测定血糖控制前后C-反应蛋白及白细胞介素6水平变化。②选择同期江苏省省级机关医院门诊体检健康人30例为正常对照组,男13例,女17例,年龄(51±7)岁,体质量指数(25.06±3.10)kg/m2,空腹及餐后2h血糖均在正常范围。均对检测指标知情同意。两组观察对象的年龄、性别构成以及身体质量指数及血压、血脂等具有可比性。③C-反应蛋白、白细胞介素6呈正偏态分布,以中位数及四分位区间表示,对各观察值取对数后进行比较分析。④各组间比较采用t检验。结果:纳入2型糖尿病患者48例,正常健康对照者30例,全部血样均进入结果分析。①糖尿病组患者治疗前后C-反应蛋白水平分别为(9.89±5.71),(6.22±4.19)mg/L,均显著高于正常对照组(2.50±1.40)mg/L,(P<0.01)。②糖尿病组患者治疗前后白细胞介素6水平分别为(13.33±4.08),(7.93±3.26)ng/L,均显著高于正常对照(3.63±2.12)ng/L,(P<0.01)。③经降糖药物治疗控制血糖后患者血清C-反应蛋白及白细胞介素6水平较治疗前显著降低(P<0.01)。结论:①2型糖尿病患者体内存在炎症因子C-反应蛋白和白细胞介素6的异常变化。②良好的血糖控制能有效降低C-反应蛋白及白细胞介素6的水平。
【Abstract】 AIM:To observe the levels of C-reactive protein(CRP)and interleukin-6(IL-6)in the newly onset type 2 diabetics and the effects of glucose control on CRP and IL-6.METHODS:①Forty-eight newly onset type 2 diabetics(22 males and 26 females)in the Department of Endocrinology,Jiangsu Provincial People’s Hospital from February to June 2003 were enrolled with the average age of(50±7)years old and body mass index(BMI)of(25.64 ±2.92)kg/m2.Knowing and agreeing with the therapeutic regimen well,all the patients whose diagnosed meeting the criterion of American Diabetes Association(ADA)took the oral hypoglycemia drug complex metformin 0.25-0.5 g,3 times a day for 3 months to control their blood glucoses.Serum levels of CRP and IL-6 were assayed with chemiluminescence immunometric assay and enzyme linked immunosorbent assay(ELISA),respectively before and after the glucose control.②Thirty healthy subjects(13 males and 17 females)on the clinic for health examination of Jiangsu Province Official Hospital worked as controls with the average age of(51 ±7)years old and BMI of(25.06±3.10)kg/m2,fasting blood glucose(FBG)and P2hBG both were in normal range.They all knew the result of identification.Good comparabilities of age,sex,BMI,blood pressure and blood lipid were available in two groups.③As both CRP and IL-6 were in positive skewness distribution,the data of CRP and IL-6 were given as median and quartile,analyzed by logarithm.④The Student’s t test analysis was used in the comparison of groups.RESULTS:All the 48 blood samples of diabetes and 30 controls were obtained and involved in the result analysis.①Serum CRP levels in T2DM were(9.89±5.71)mg/L,(6.22±4.19)mg/L respectively,both significantly higher than those in normal control group(2.50±1.40)mg/L(P < 0.01).②Serum IL-6 levels in T2DM were(13.33±4.08)ng/L,(7.93±3.26)ng/L,respectively,both remarkably higher than those of normal controls(3.63 ±2.12)ng/L(P < 0.01).③Both levels of CRP and IL-6 were markedly decreased in T2DM after treatment with complex metformin following the improvement of hyperglycemia(P < 0.01).CONCLUSION:①Inflammatory factors such as CRP and IL-6 shows abnormality in the newly onset type 2 diabetics.② Good glucose control can both significantly decrease levels of CRP and IL-6.
