哈萨克族原发性高血压伴肥胖患者血脂、血糖及血清尿酸分析
Levels of serum lipid,glucose and uric acid in Kazakstan patients with essential hypertension accompanying obesity
【摘要】 目的:分析哈萨克族原发性高血压伴肥胖患者血脂、血糖及血清尿酸的水平。方法:于2003-06/2004-10选择哈萨克族原发性高血压患者76例及与哈萨克族原发性高血压患者同居住区的哈萨克族非高血压人群83例为研究对象。根据体质量指数(≥25kg/m2为肥胖)分为高血压伴肥胖组40例、高血压无肥胖组36例、肥胖对照组40例、无肥胖对照组43例。取清晨空腹静脉血,测定血清总胆固醇、三酰甘油、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇、血尿酸及血糖。结果:纳入研究对象159例,均进入结果分析。①高血压伴肥胖组总胆固醇、三酰甘油、低密度脂蛋白胆固醇、尿酸较高血压无肥胖组及无肥胖对照组显著升高[总胆固醇分别为(5.24±0.87),(4.76±1.06),(4.24±0.55)mmol/L,三酰甘油分别为(1.72±0.42),(1.31±0.50),(1.18±0.28)mmol/L,低密度脂蛋白胆固醇分别为(3.01±0.71),(2.70±0.74),(2.32±0.37)mmol/L,尿酸分别为(268.55±74.22),(220.06±59.51),(200.63±55.30)μmol/L,P<0.05或P<0.01]。②高血压伴肥胖组血糖较无肥胖对照组也显著升高[分别为(5.67±0.58),(5.39±0.57)mmol/L,P<0.05]。③高血压伴肥胖组血脂、血糖、血尿酸与肥胖对照组相比差异无显著性意义(P>0.05)。④高血压无肥胖组总胆固醇、低密度脂蛋白胆固醇与无肥胖对照组比较显著升高(P<0.01)。⑤肥胖对照组总胆固醇、三酰甘油、低密度脂蛋白胆固醇、尿酸与无肥胖对照组比较也显著升高[总胆固醇分别为(5.13±1.04),(4.24±0.55)mmol/L,三酰甘油分别为(1.60±0.68),(1.18±0.28)mmol/L,低密度脂蛋白胆固醇分别为(3.12±0.90),(2.32±0.37)mmol/L,尿酸分别为(242.15±77.69),(200.63±55.30)μmol/L,P<0.01]。结论:哈萨克族原发性高血压伴肥胖患者存在血脂、血糖及血尿酸水平升高等代谢紊乱的倾向。
【Abstract】 AIM: To explore the levels of serum lipid, glucose and uric acid in patients of Kazakstan with essential hypertension accompanying obesity. METHODS: Totally 76 Kazakh with hypertension and 83 Kazakh without hypertension but inhabited in the same district were selected between June 2003 and October 2004. They were divided into 4 groups according to their body mass index (≥ 25 kg/m2 was defined as obesity): hypertension accompanying obesity group (n=40), hypertension without obesity group (n =36), obesity control group (n=40) and non-obesity control group (n=43). Fasting venous blood was drawn in the morning to detect the levels of total cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), blood uric acid and glucose in serum. RESULTS: All the 159 subjects were involved in the result analysis. ①The levels of serum TC, TG, LDL-C and blood uric acid in the hypertension accompanying obesity group were significantly higher than those in the hypertension without obesity group and non-obesity control group [TC: (5.24±0.87), (4.76±1.06), (4.24±0.55) mmol/L; TG: (1.72±0.42), (1.31±0.50), (1.18±0.28) mmol/L; LDL-C: (3.01±0.71), (2.70±0.74), (2.32 ±0.37) mmol/L; uric acid: (268.55±74.22), (220.06±59.51), (200.63 ±55.30) μmol/L, P < 0.05 or P < 0.01]. ②The level of blood glucose in the hypertension accompanying obesity group was significantly increased compared with that in the non-obesity control group [(5.67±0.58), (5.39±0.57) mmol/L, P < 0.05]. ③There was no significant difference in the levels of serum lipids, glucose and uric acid between the hypertension accompanying obesity group and the obesity control group (P > 0.05). ④Compared with the non-obesity control group, the serum levels of TC and LDL-C in the hypertension without obesity group were significantly higher (P < 0.01). ⑤The levels of serum TC, TG, LDL-C and blood uric acid in the obesity control group were significantly higher than those in the non-obesity control group [TC: (5.13±1.04), (4.24±0.55) mmol/L; TG: (1.60±0.68), (1.18±0.28) mmol/L; LDL-C: (3.12±0.90), (2.32±0.37) mmol/L; uric acid: (242.15±77.69), (200.63±55.30) μmol/L, P < 0.01]. CONCLUSION: There is a metabolic disorder tendency such as the increase of serum lipids, glucose and uric acid in Kazakstan patients with hypertension accompanying obesity.
