骨髓基质细胞移植治疗缺血性心脏病:影像学及间接免疫荧光法评估
Marrow stromal cell transplantation for ischemic heart disease An evaluation of imaging and indirect immunofluorescence
【摘要】 目的:观察超声成像及间接免疫荧光技术在骨髓基质细胞移植治疗缺血心肌病方面的评估价值。方法:试验于2004-10/2005-10在武汉大学医学院心血管病研究所完成。选取20只大耳白兔开胸结扎左冠状动脉前降支建立心肌梗死模型后,存活18只,将其随机分为对照组和移植组两组。分别在造模前1天、造模后1周、骨髓基质细胞移植后4周行超声检查,测量左室前壁厚度、左室舒张末期内径、射血分数,左室前壁收缩期峰值速度、舒张早期峰值速度、舒张晚期峰值速度。对BrdU标记的移植细胞行抗肌钙蛋白T和Ⅷ因子免疫荧光检测,探讨其分化和转归。结果:20只大耳兔于造模后死亡2只,进入结果分析18只。①超声检查:移植组移植4周后左室舒张末期内径较对照组明显缩小[(12.2±1.9,14.9±2.3)mm,t=2.641,P<0.05]。移植组移植4周后射血分数、左室前壁收缩期峰值、舒张早期峰值速度明显高于对照组[(67.3±10.2,54.6±9.6)%,(10.2±1.9,7.0±2.1)cm/s,(10.9±2.7,7.9±2.6)cm/s]。②免疫荧光检测:部分移植细胞抗心肌特异肌钙蛋白T染色阳性,显示清晰的心肌横纹表明向心肌细胞分化;另有部分细胞Ⅷ因子为阳性,证实成功分化成为内皮细胞。结论:超声能够检测到兔梗死心肌MSCs移植后心肌局部运动与心脏功能的改善,其机制与其向心肌细胞及内皮细胞分化有关。
【Abstract】 AIM To observe the evaluation value of ultrasonic imaging and indirect immunofluorescence technique in the treatment of ischemic cardiomyopathy with marrow stroma cells MSCs transplantation. METHODS The experiment was performed at the Cardiovascular Institute Medical College Huhan University from October 2004 to October 2005. Totally 20 flap-eared rabbits were selected and received left anterior descending LAD ligation to establish myocardial infarction models. The 18 survival rabbits were randomly assigned into control group and transplantation group. Ultrasonic examination was performed at day 1 before week 1 after establishing models and week 4 after MSCS transplantation. Depth of anterior wall of left ventricle inner diameter and ejection fraction in diastasis of left ventricle peak systolic flow velocity of anterior wall of left ventricle peak early diastolic velocity and peak late diastolic velocity were measured. BrdU labeled transplantation cells were subjected to anti-troponin T and Ⅷ factor immunofluorescence determination and then explored its differentiation and turnover. RESULTS Of the 20 flap-eared rabbits 2 rabbits died after establishing models so 18 rabbits entered the result analysis. ①ultrasonic examination Inner diameter in diastasis of left ventricle 4 weeks after transplantation in the transplantation group became short markedly as compared with the control group (12.2±1.9,14.9±2.3)mm,t=2.641P < 0.05.Ejection fraction peak systolic flow velocity of anterior wall of left ventricle and peak early diastolic velocity 4 weeks after transplantation in the transplantation group were remarkably higher than that in the control group (67.3± 10.2,54.6±9.6)%,(10.2±1.9,7.0±2.1)cm$s(10.9± 2.7,7.9±2.6)cm$s. ②immunofluorescence determination Anti-troponin T of partial transplantation cells stained positively. The clear myocardial striations showed that it differentiated into myocardial cells. Ⅷ factor of partial cells stained positively which verified it differentiated successfully into endothelial cells. CONCLUSION Ultrasound can found the local movement of myocardium and improvement of cardiac function after MSCs transplantation of rabbits’ infarcted myocardium. Its mechanism is associated with the differentiation into myocardial cells and endothelial cells.
