99Tcm-MIBI与13N-NH3心肌灌注显像评价兔亚急性心肌梗死缺血灶的准确性比较

【摘要】 目的:比较99Tcm-MIBI单光子计算机断层显像(SPECT)与13N-NH3正电子断层显像(PET/CT)两种心肌灌注显像方法,对兔亚急性心肌梗死灶显像的准确性。方法:实验于2003-10/2004-03在解放军第四军医大学西京医院核医学科3实验室进行。取8只雄性新西兰大白兔,结扎其冠状动脉左前降支,45min后松解形成再灌注。饲养7d后,再次结扎45min松解。2h后分别行99Tcm-MIBISPECT与13N-NH3PET/CT两种心肌灌注显像,通过定量化数据转化,计算心肌梗死灶缺损百分率,与TTC组织染色得出心肌梗死灶缺损百分率相比较。结果:8只兔全部进入结果分析。心肌梗死灶TTC染色后显示缺损度为(24.2±1.9)%,13N-NH3PET/CT显像显示缺损度为(23.7±2.3)%,两者均显著高于99Tcm-MIBISPECT心肌灌注显像(20.5±2.5)%,P<0.05。结论:13N-NH3PET/CT心肌灌注显像能更准确反映心肌梗死灶的实际范围。

【Abstract】 AIM: To compare the accuracy of myocardial perfusion imaging between 99Tcm-MIBI single photon emission computerized tomograph (SPECT) and 13N-NH3 positron emission tomography/computerized tomography (PET/CT)in rabbits with subacute myocardial infarction. METHODS: The experiment was conducted at the Laboratory of Nuclear Medicine, Xijing Hospital, Fourth Military Medical University of Chinese PLA between October 2003 and March 2004. Eight male New Zealand White rabbits of which left anterior descending (LAD) of coronary arteries were completely occluded for 45 minutes followed by reperfusion. One week later, the rabbits were ligated again for 45 minutes. Two hours later, myocardial perfusion imagings were performed on 99Tcm-MIBI SPECT and 13N-NH3 PET/CT, respectively. Defect percentage of myocardial infarction heart was calculated by quantitative data transformation, and compared with the defect percentage of myocardial infarction heart determined by triphenyl tetrazolium chloride (TTC) staining. RESULTS: Totally 8 rats were involved in the result analysis. Infarct size were (24.2±1.9)% by TTC staining, while (23.7±2.3)% by 13N-NH3 PET/CT, and the both were significantly higher [(20.5±2.5)%,(P < 0.05)] by 99Tcm-MIBI SPECT. CONCLUSION: The 13N-NH3 PET/CT myocardial perfusion imaging can more accurately reflect the infarct size.
