不同病因慢性心力衰竭心肌纤维化研究
The Study of Myocardial Fibrosis of Chronic Heart Failure
【摘要】 目的:探讨慢性心力衰竭的心肌纤维化及干预治疗。方法:采用放射免疫分析法,测定76例慢性心力衰竭(心衰)患者(冠心病组31例,风湿性心脏病组23例,扩张型心肌病组22例)血管紧张素Ⅱ(AngⅡ)、醛固酮(ALD)、Ⅰ型前胶原氨端肽原(PⅠNP)、Ⅲ型前胶原氨端肽原(PⅢNP)含量。并与20例健康对照组进行比较。结果:风心病组、扩张型心肌病组AngⅡ含量显著高于正常对照组。各心衰组血浆ALD和PⅠNP含量均显著高于正常对照组。风心病组、扩张型心肌病组PⅢNP含量显著高于正常对照组。扩张型心肌病组Ⅰ/Ⅲ型胶原比值显著高于正常对照组。结论:慢性心力衰竭患者胶原合成增加,血中AngⅡ、ALD在胶原沉积过程中具有重要作用。不同病因心衰患者均应给予抗纤维化治疗。
【Abstract】 Objective: To evaluate the myocardial fibrosis in chronic heart failure(CHF)patients and intervention treatment. Methods: The serum levels of PⅠNP, PⅢNP, AngⅡ, and ALD were measured by radioimmunoassay in seventy-six CHF patients(the CHD group of 31 cases, the rheumatic heart group of 23 cases, the dilated cardiomyopathy group of 22 cases) and compared with 20 healthy controls. Results: The serum levels of AngⅡ in rheumatic heart disease and dilated cardiomyopathy were significantly higher than those of the control group.ALD and PⅠNP were drastically higher in all CHF patients than those in the control group. The serum levels of PⅢNP were significantly higher in the CHF patients of rheumatic heart disease and dilated cardiomyopathy than those in the control group. The Ⅰ/Ⅲ collagen ratio was significantly higher in the dilated cardiomyopathy group than that in the control group. Conclusion: There is myocardial collagen fibrosis in all CHF patients. The AngⅡ and ALD play important roles on deposit of cardiac interstitial collagen. It is necessary to treat with the therapy of antifibrosis on the patients with CHF.
