HPV感染、P53及Survivin蛋白表达与非小细胞肺癌的相关性
Infection of HPV and Its Relationship with Expression of P53 and Survivin in NSCLC
【摘要】 目的探讨人乳头瘤病毒(HPV)感染在非小细胞肺癌(NSCLC)发生中的病因学意义,并研究HPV感染与细胞凋亡、P53及Survivin蛋白表达之间的关系。方法分别应用PCR、TUNEL和免疫组化法检测76例NSCLC及13例肺良性病变中HPV DNA、凋亡指数(AI)、P53和Survivin蛋白表达情况。结果肺癌组HPV DNA检出率为40.8%(31/76),明显高于肺良性病变组[7.7%(1/13)],两者差异有显著性意义(P<0.05)。高、中分化肺癌HPV感染率明显高于低分化肺癌,吸烟者明显高于不吸烟者(均P<0.05)。NSCLC中P53和Survivin蛋白表达阳性率为63.2%(48/76)和42.1%(32/76),肺良性病变组均未见表达。NSCLC中HPV DNA阳性组P53蛋白表达阳性率明显高于阴性组(P<0.05)。P53蛋白表达阳性组中Survivin蛋白表达阳性率明显高于阴性组(P<0.05)。NSCLC中HPV DNA、P53和Survivin蛋白表达阳性组与阴性组之间的凋亡指数均有显著性差异(P<0.05)。结论HPV感染可能是NSCLC发生的重要因素之一,HPV感染可能导致P53基因突变,突变的P53使Survivin蛋白表达增加,抑制凋亡,促进NSCLC的发生。
【Abstract】 Objective To investigate the role of human papillomavirus(HPV) infection in the pathogenesis of non-small cell lung cancer(NSCLC) and to elucidate the possible relationship between HPV infection and cell apoptosis,P53 and Survivin expression.Methods PCR,TUNEL and immunohistochemistry were applied to detect HPV DNA,apoptosis index(AI),P53 and Survivin expression in 76 NSCLC and 13 normal lung specimens.Results HPV DNA was detected in 40.8%(31/76) of lung cancer specimens and 7.7%(1/13) of benign disease specimens by PCR.There was a significant difference between the two groups.HPV infection rate in well and moderately differentiated NSCLC was significantly higher than in the worse differentiated NSCLC,and that in smokers with NSCLC was significantly higher than in non-smokers.Expression rate of P53 and Survivin protein was 63.2%(48/76) and 42.1%(32/76) respectively in NSLCL,and no expression was detectable in benign disease specimens.The positive rate of P53 expression was significantly higher in NSCLC patients positive for HPV than in negative cases.The positive rate of Survivin expression was significantly higher in P53 positive group than in P53 negative group.There was statistically significant difference in AI between HPV DNA,P53 and Survivin expression positive group and negative group in NSCLC.Conlusion HPV infection may play an important role in the pathogenesis of NSCLC.HPV infection may induce P53 mutation.Mutative P53 promoted the Survivin expression,inhibited the apoptosis and accelerated the occurrence of NSLCL.
