体外循环围术期患者血清补体C3、C4水平变化及临床意义
The Clinical Value of Dynamic Determination of Serum Complement C3 and Complement C4 during Perioperation of Cardiopulmonary Bypass Surgery
【摘要】 目的探讨在体外循环(cardiopulmonary bypass,CPB)下心脏手术围术期检测血清补体C3、C4水平变化的临床意义。方法选择20例进行CPB手术和20例非CPB手术围术期中几个时期的血液(包括术前、术后即刻、术后24 h、术后48 h和术后72 h),测定其血清补体C3、C4的水平。结果CPB组患者血清补体C3水平在CPB停流转后显著低于术前水平(0.67±0.29 g/Lvs1.36±0.61 g/L,P<0.01),术后2 d恢复并显著高于术前水平(1.71±0.51 g/Lvs1.36±0.61 g/L,P<0.01);血清补体C4水平在CPB停流转后显著低于术前水平(0.14±0.07vs0.27±0.16 g/L,P<0.01),术后3 d恢复并显著高于术前水平(0.35±0.18 g/Lvs0.27±0.16 g/L,P<0.01);对照组各项指标较术前均无显著变化(P>0.05)。结论CPB围术期观察血清补体C3、C4水平对心脏外科临床有着指导作用。
【Abstract】 Objective To explore the clinical value of the determination of the kinetic levels of serum complement 3(C3) and complement 4(C4) during perioperation of cardiopulmonary bypass(CPB) surgery.Methods C3 and C4 were determined in 20 patients with heart diseases and 20 patients with other thoracic diseases before and after the surgery(24 h,48 h and 72 h respectively).Results C3 decreased markedly after CPB cardiac surgery,its levels being lower significantly than those before the surgery in CPB group (0.67±0.29 g/L vs 1.36±0.61 g/L,P<0.01),and the levels of C3 returned to certain range 48 h after operation which were even much higher than those before the surgery(1.71±0.51 g/L vs 1.36±0.61 g/L,P<0.01).C4 also decreased markedly after the surgery,its levels being lower significantly than those before the surgery in CPB group(0.14±0.07 vs 0.27±0.16 g/L,P<0.01),and the levels of C4 returned to certain range 72 h after operation which was even much higher than those before the surgery(0.35±0.18 g/L vs(0.27)±0.16 g/L,P<0.01).There was no difference significantly in these parameters in control group(P>0.05).Conclusion C3 and C4 are of definite significance in monitoring clinical state in patients during perioperation of CPB surgery.
