【摘要】 目的探讨多指挤压旋转撕脱性离断再植方法的选择与临床效果。方法1997年8月～2005年8月,对多指挤压旋转撕脱性离断采用不同方法再植11例22指,其中A组应用自体小静脉移植7例14指,B组采用掌(指)固有动脉移位4例8指。结果自体小静脉移植组成活13指;掌(指)固有动脉移位组全部成活。两组病例术后平均随访5.5个月(4～12个月),按中华医学会手外科学会断指再植功能评定试用标准评定,两种手术方法优良率分别为84.6%、87.5%。两组再植指成活率无明显差异(x2=0.08,P>0.05),术后功能优良率也无明显差异(x2=0.14,P>0.05)。结论多指挤压旋转撕脱性离断再植方法各异,正确判断伤情,选择最佳手术方案是再植成功的关键。自体小静脉移植和掌(指)固有动脉移位再植,是两种实用有效的再植方法。更多还原

【Abstract】 Objective To investigate the replantation methods and treatment outcome of severed fingers with crushed and rotation avulsion.Methods From August 1997 to August 2005, 11 cases with 22 multi-severed fingers which was crushed and rotation avulsion were replanted by different approaches. 7 cases with 14 amputated fingers(A group)were replanted by auto-innominate vein graft, 4 cases with 8 amputated fingers(B group)were replantated by replacement of natural- arteria metacarpea or natural-arteria digitalis. Results 7 cases with 13 amputated fingers were survival in the A group, All amputated fingers were survival in the B group. Patients were followed up 5.5 months on averagerange 3-12 months,According to the criteria for functional assessment of amputated finger issued by the Branch of Hand Surgery of Chinese Medicine Association,the excellent and good rate of the A group was 84.6% and the B group was 87.5%.The difference of survival rate and excellent rate between two groups was not statistically significant.Conclusion It is an effective and practical method in replantation of multi-severed fingers with crushed and rotation avulsion by auto- innominate vein graft and replacement of natural-arteria metacarpea or natural- arteria digitalis.更多还原
