重症脑血管病患者连续脑电图构型研究
Clinical research of the CEEG patterns of patients with critical cerebrovascular disease
【摘要】 目的应用连续脑电图监测重症脑血管病患者,观察其表现模式及其与预后的关系。方法收集神经内科重症监护病房的住院患者42例,其中男性27例,女性15例,年龄47~87岁,平均年龄69.21岁;脑梗死16例,脑出血26例,均为有意识障碍且昏迷者,格拉斯哥昏迷评分<8分。给予连续脑电图检查,按昏迷的脑电图模式分析脑电图构型。结果42例重症脑血管病患者的连续脑电图全部异常。表现为弥散性慢活动13例,共3例死亡;间歇性δ节律3例,无死亡病例;癫痫样活动9例,4例死亡;三相波2例,1例死亡;爆发-抑制模式1例,死亡;单节律活动3例,均死亡;6例表现为低波幅活动,全部死亡;局灶性异常5例,无死亡。结论重症脑血管病患者的连续脑电图表现主要有:弥散性慢活动、间歇性δ节律、癫痫样活动、三相波、爆发-抑制、单节律活动(α昏迷、θ昏迷)低波幅活动及局灶性异常。弥散性慢活动、间歇性δ节律及局灶性异常预后较好,低波幅活动、单节律活动、癫痫样活动预后较差。
【Abstract】 Objectives To investigate the CEEG patterns of patients with critical cerebrovascular disease and its relationship with the prognosis.Methods Forty-two unconscious patients(Glasgow coma score<8), including 16 patients with cerebral infarction and 26 patients with intracerebral hemorrhage, were treated in neurological intensive care unit from March 2004 to March 2005. All of the patients underwent CEEG monitoring.Results The CEEG of the 42 patients with critical cerebrovascular disease was markedly abnormal; Diffuse slowing activity was found in 13 patients, of which 3 patients died; intermittent delta rhythms in 3 patients, all of which survived; epileptiform activities in 9 patients, of which 4 patients died; triphasic waves in 2 patients, of which 1 patient died; burst-suppression pattern in 1 patient, who died; monorhythmical activities in 3 patients, all of which died; low voltage output EEG in 6 patients, all of which died; focal abnormalities in 5 patients, all of which survived.Conclusion The CEEG patterns of patients with critical cerebrovascular disease are as follows: diffuse slowing activity, intermittent delta rhythms, epileptiform activities, triphasic waves, burst-suppression pattern, monorhythmical activities (alpha coma, theta coma) , low voltage output EEG and focal abnormalities. Diffuse slowing activity, intermittent delta rhythms, focal abnormalities are associated better prognosis, whereas low voltage output EEG, monorhythmical activities and epileptiform activities are associated worse prognosis.
