【摘要】 目的探讨高龄结肠癌伴急性肠梗阻患者的外科治疗原则和手术方式的选择。方法回顾1993年至2002年高龄结肠癌伴急性肠梗阻患者共计79例,78例行手术治疗。结果一期切除吻合术47例,其中根治性切除术11例,肿瘤楔形切除术36例;Hartm ann手术23例,单纯结肠造口术5例,回肠-横结肠吻合术2例,剖腹探查术1例。中高分化腺癌61例,低分化腺癌10例,黏液腺癌8例。Duke’s A期1例,B期37例,C期29例,D期12例。结论高龄结肠癌伴急性肠梗阻患者需根据患者的具体情况决定手术方式,有一期手术和分期手术多种,应根据高龄患者的特点有针对性地加强围手术期处理。更多还原

【Abstract】 Objective To discuss the principals and methods of surgical treatment for advanced age patients with colon cancer complicated with acute obstruction.Methods Seventy nine advanced age patients with colon cancer complicated with acute obstruction diagnosed from 1993 to 2002 were analyzed retrospectively.Seventy eight of them were treated by operation.Results Forty seven patients received one-stage operation,including 11 radical excision,36 wedge resection;23 Hartmann operation;5 colostomy;two ileocolic anastomosis and one exploratory laparotomy.Sixty one patients were with well-differentiated or moderately differentiated adenocarcinoma,10 with poorly differentiated adenocarcinoma and 8 were with mucinous adenocarcinoma.Conclusion The choice of operation for the advanced age patients with colon cancer complicated with acute obstruction is dependent on the specific condition of the patients.There are several choices of operation,such as on-stage operation and staging operation.On the basis of the characteristic of advanced age patients,the perioperative management should be emphasized.更多还原
