乳腺癌钼靶X线微钙化与癌细胞ER、PR表达关系的研究
Study on the Relationship Between Malignant-appearing Microcalcification in the Mammography and Estrogen/Progesterone Receptor Status
【摘要】 目的分析钼靶片中乳腺癌微钙化与ER、PR表达及临床病理关系,评价微钙化征象预测ER、PR表达的可行性。方法乳腺癌病例115例,钼靶X线片由3位有经验的影像诊断医师阅片,明确钙化并根据钙化及相关表现分组。免疫组化采用SP法,根据癌细胞核内染色判断ER、PR表达情况。各钙化组分别与ER、PR表达情况进行比较分析。结果乳腺癌钼靶片中微钙化的有无、微钙化数目、钙化类型与肿瘤细胞ER、PR表达无明显相关性(P>0.05)。乳腺癌微钙化多见于导管癌,占53.33%,是钼靶片中无肿块表现乳腺癌的主要X线征象。微钙化表现与乳腺癌临床TNM分期无关(P>0.05)。乳腺癌钼靶片微钙化表现常伴腋下淋巴结转移(24/47例),而钙化数目较少(<20枚)的乳腺癌更易伴有腋下淋巴结转移(16/24例)。结论微钙化在钼靶片乳腺癌诊断中,特别是在早期癌以及无肿块表现的乳腺癌诊断中具有重要意义。但是,微钙化表现与ER、PR表达无明显相关性,尚不能作为乳腺癌ER、PR表达的预测指标。
【Abstract】 Objective To explore the putative relationship between the mammographic malignant-microcalcification and the expressionof Estrogen and Progesterone recepters and to evaluate the feasibility of predicting ER,PR status by mammographic microcalcification.Methods 115 cases of breast cancer with mammographic and corresponding pathological data were included in this study.Mammographic microcalcification were determined by three radiologists.ER,PR expression of the breast carcinomas was determined with immunohistochemical staining technique.The relationship between mammographic microcalcification features and nuclear ER,PR expression was statistically analyzed.Results No statistically significences were found between different mammographic microcalcification features and ER,PR expression in the breast carcinomas(P>0.05).Microcalcification was frequently found in ductal carcinomas among different histological types(53.33%).Microcalcification was the major X-ray feature of breast cancer in the no-mass mammography.No relationship between microcalcification and TNM grades were found(P>0.05).Microcalcification was grequently found in the cases with axillary lymph nodes metastases(24/47,51.06%),especially,that with fewer microcalcifications(<20,16/24,66.67%).Conclusion Microcalcification is of significant value in the diagnosis of breast cancer,especially early breast cancer or those showing no-mass in mammography.No correlation is found between mammographic microcalcification and ER,PR expression in the breast cancer and it is not a reliable method to predict homone receptors status on the basesis of microcalcification for clinical purposes.
