扩散张量成像及纤维追踪成像对原发性进行性失语脑内联合纤维束研究
Study on Association Fasciculi of Primary Progressive Aphasia by Diffusion Tensor Imaging and Fiber Tractography Techniques
【摘要】 目的应用扩散张量成像(DTI)及纤维追踪成像(F iber tractography)检查结果,揭示原发性进行性失语(PPA)患者大脑半球内、半球间联系纤维结构改变。方法1例男性,56岁,语言能力进行性下降3年。SIEMENS Trio 2003T及工作站完成全脑MR I和DTI数据收集和后处理。结果常规MR I显示左侧额叶、颞叶轻度萎缩。部分各向异性(FA)图显示左侧海马、外囊较对侧小。左侧弓状纤维束、下纵行纤维束、扣带束、海马及外囊等主要的联合、连合纤维均较对侧减少,左侧小钳变小。结论PPA患者除语言功能区的纤维结构改变外,其脑内主要联合、连合纤维也较正常人减少。
【Abstract】 Objective To exhibit the changes of association fasciculi of primary progressive aphasia(PPA) with diffusion tensor imaging (DTI) and fiber tractography technique.Methods Conventional MRI,DTI and fiber tractography were performed in a 56 years old man with PPA by 3.0 T SIEMENS Trio 2003T MRI.Seeds track fiber bundles.Especially DT Fiber tractography depicted hippocampus,extra-capsule,arcuate fasciculi,cingulum,inferior longitudinal fasciculi and corpus callosum.Results Conventional MRI demonstrated mild atrophy of left temporal lobe and frontal lobe.Fractional anisotropy(FA) maps showed white matter volume of hippocampus and extra-capsule were thinner than contralateral.Fiber bundles of left arcuate fasciculi,cingulum,inferior longitudinal fasciculus,hippocampus,and extra-capsule were reduced.Left frontal fasciculi of corpus callosum decreased.All of their mean FA and neural fasciculi also decreased than health.Conclusion The white matter changes in PPA are not only language areas fibers decrease,but also association fasciculi reduce,too.
