【摘要】 目的探讨经腹根治性子宫颈切除术治疗有生育欲望的早期浸润性子宫颈癌患者的可行性与安全性。方法2004年4月~2005年3月,对3例Ⅰa期和2例Ⅰb期子宫颈癌患者行经腹根治性子宫颈切除术。结果5例患者均成功施术,其中1例因术中冷冻切片检查发现癌灶距宫颈切缘仅2 mm,且已切除全部宫颈,遂补行子宫体切除术;1例术后病理检查发现脉管内癌栓而术后辅助行TP方案化疗2个周期。平均手术时间为202 min(180~230 min),术中平均出血量为325 ml(150~650 ml),术中、术后无并发症。随访12~22月,无肿瘤复发,4例患者月经正常,1例妊娠36+5周者经剖宫产术娩出一健康女婴,现母婴均健康。结论只要严格掌握适应证,经腹根治性子宫颈切除术治疗有生育欲望的早期浸润性子宫颈癌患者是安全、可行的。更多还原

【Abstract】 Objective To explore the safety and reliability of transabdominal radical trachelectomy for the treatment of early-stage cervical carcinoma.Methods Between April 2004 to march 2005,5 patients with early-stage cervical carcinoma,which 3 were stage Ⅰa and 2 were stage Ⅰb,were planed to undergo transabdominal radical trachelectomy for preserve fertility.Results All 5 patients were successfully perform the surgery of transabdominal radical trachelectomy.1 case was corpus uterus resection additionally for the lesion only 2mm apart from the cut margin,1 patient was deliver 2 cycles complemental chemotherapy of TP scheme.The mean operative time was 202 minutes（180～230 minutes）.The mean blood loss was 325 mL（150～650 mL）.No complications were found in all patients.After 12～22 months of follow-up,no tumor relapsed,4 patients had normal menstruation,1patient had a delivery after pregnancy of 36⁺⁵ weeks.Conclusion Transabdominal radical trachelectomy was a feasible surgery for the patients with early-stage invasive cervical carcinoma who wish to preserve fertility.更多还原
