计算机辅助P波离散度测定及其对阵发性心房颤动的预测价值

【摘要】 目的研究一种快速准确的P波离散度(Pd)计算机辅助测算方法,评价Pd对阵发性心房颤动(房颤)的预测价值。方法将12导联同步描记心电图进行计算机数字化处理,每导联各叠加3个心动周期的P-QRST波群,将振幅放大至正常心电图的10倍,显示在计算机屏幕上,手工确定P波起始和终末点,由计算机自动测算出Pmax、Pmin和Pd。对43例阵发性房颤病人和46例健康查体者进行对比分析。结果计算机辅助测算Pd所需时间为(48±6)s,手工测算Pd所需时间为(480±60)s,两者比较差异具有显著意义(t=46.97,P<0.01)。房颤组Pd为(53.3±7.4)ms,对照组为(25.7±7.8)ms,房颤组Pd较对照组明显延长(t=17.1,P<0.01)。结论获得了一种快速准确的Pd计算机辅助测算方法,计算机辅助Pd测定对阵发性房颤具有一定的预测价值。

【Abstract】 Objective To investigate a fast and accurate computer-assisted detection of P wave dispersion(Pd),and to evaluate its value in predicting paroxysmal atrial fibrillation.MethodsThe signals of 12-lead electrocardiogram(ECG) were sampled and digitized by P586 personal computer.The P-QRST waveforms of three cardiac cycles were added and magnified to 10 times of normal ECG,then displayed on the screen.The origin and terminal points of P waveform were detected manually,and Pmax,Pmin and Pd measured automatically by computer.A matched control analysis was made in 43 patients and 46 controls of non-atrial-fibrillation.ResultsThe time of Pds calculated by computer was shorter than that of routine manual method(48±(6 s) vs(480±)60 s,t=46.97,P<0.01);Pds of the patients with paroxysmal atrial fibrillation were longer than that of non-atrial-fibrillation(53.3±7.4 ms vs 25.7±7.8 ms,t=17.1,P<0.01).ConclusionA fast and accurate computerized measurement of Pd is obtained,which is of value in predicting paroxysmal atrial fibrillation.
