【摘要】 目的:探讨乙型肝炎病毒(HBV)感染孕妇在早孕期开始使用HBV高效免疫球蛋白(HB IG)的干预效果。方法:选取早孕时血清HBV-DNA(+)的孕妇200例,随机分为研究组和对照组各100例,研究组于孕12周起,对照组于孕28周起,每月1次肌注HB IG 400 IU至分娩。于孕12周和分娩时定量检测孕妇血清HBV-DNA,定性检测脐带血HBV-DNA和抗-HB s的阳性率。结果:研究组和对照组孕妇在孕12周和分娩时血清HBV-DNA的含量、新生儿的脐血HBV-DNA和抗-HB s的阳性率对比分别为6.73±2.15 V S 6.49±1.87(P>0.05),3.62±2.75 V S 5.01±1.79(P<0.05);9%V S 19%(P<0.05);25%V S 13%(P<0.05)。早使用HB IG的研究组孕妇血HBV-DNA的含量及其新生儿的宫内感染率较对照组显著低。两组孕妇及其新生儿未发现有不良反应。结论:孕期尽早使用HB IG安全有效,可进一步减少孕妇血清HBV-DNA的含量和HBV的宫内传播率。更多还原

【Abstract】 Objective: To evaluate the interruptive effect of hepatitis B virus(HBV) intrauterine transmission by means of early using of HBV immunoglobulin(HBIG).Methods: 200 serologic HBV-DNA(+) pregnant women were randomly divided into study group and control group.Each subject in study group and control group received HBIG 400 IU intramuscularly per month before delivery since 12th and 28th weeks of gestation respectively.The quantity of HBV-DNA in the peripheral blood of the women at delivery and 12th weeks of gestation were measured by polymerase train reaction(PCR),meanwhile,the positive rate of HBV-DNA and anti-HBs in the cord blood were detected by PCR and ELISA respectively.Results: Of the study group and control group,the positive rate of HBV-DNA and anti-HBs in the cord blood were 9% VS 19%(P<0.05) and 25% VS 13%(P<0.05) respectively;the HBV-DNA load in the maternal peripheral blood before and after the HBIG administration were 6.73±2.15 VS 6.49±1.87(P>0.05),(3.62)±2.75 VS 5.01±1.79(P<0.05),respectively.No side effect was found in all the neonates or women.Conclusion: The HBV transmission in uterus may be interrupted more effectively by the early using of HBIG without any side effect,the HBV infected pregnant women should received HBIG 400 IU intramuscularly per month as early as they can.更多还原
