早期类风湿关节炎寒热证候分类与细胞因子及有关临床指标的相关性研究

【摘要】 目的:探索早期类风湿关节炎(RA)寒、热证候分类的客观基础,研究中医四诊信息与RA细胞因子、临床炎性指标、临床免疫学指标的相关性。方法:随机选取符合纳入标准的早期RA女性患者70例,按RA证候问卷表采集患者各项证候信息,参照相应标准进行寒、热证候判断,活动性判断。同时,取患者晨起空腹血样,检测细胞因子、临床炎性指标和临床免疫学检查指标。采用两因素方差分析为主的显著性检验方法,在控制RA活动性的基础上,分析各客观指标在寒、热证候之间的差异。结果:除C反应蛋白(CRP)外,早期女性RA寒热证候患者细胞因子(IL-8、TNF-α、IL-10),临床炎性检查指标(ESR),及各免疫学指标(IgA、IgM、IgG、RF、C3)的值未见显著性差异。结论:早期RA患者寒热证候之间CRP有显著性差异,寒证的CRP低于热证,提示CRP可以作为RA寒热证候分类的微观指标之一。

【Abstract】 Objective: To evaluate the diagnositic value of the cytokines (TNF-α, IL-10, IL-8), clinical inflammatory indexes (ESR, CRP), and clinical immune indexes (IgA, IgM, IgG, RF, C3) on heat or cold syndrome in female patients with rheumatoid arthritis(RA) of early stage. Methods: 70 female RA patients were divided into heat and cold syndrome type. The blood samples were collected and detected for the following indexes such as cytokines, clinical inflammatory indexes, clinical immune indexes. The results were analyzed by two-way classification ANOVA. Results: Except CRP, the expression of cytokine (TNF-α, IL-10, IL-8), clinical inflammatory indexes (ESR), immune indexes (IgA, IgG, IgM, RF, C3)of subjects with heat and cold syndrome had no significant difference. Conclusion: Female RA patients with heat syndrome have higher expression of CRP than those with cold syndrome, which indicates that CRP has potential diagnositic value to heat and cold syndrome in RA.
