ADVIA 120型血液分析仪异常报警参数及其可靠性分析

【摘要】 目的了解ADVIA120型全自动血液分析仪对细胞形态异常提示功能的可靠性。方法将160例仪器提示异常参数的标本和112例无异常提示的标本作血涂片,经瑞特-姬姆萨染色镜检后比较结果。结果真阳性率:未成熟粒细胞(IG)、血红蛋白浓度不等(HC-VAR)、小红细胞(M ICRO)、核左移(LS)、大红细胞(MAC-RO)、低色素性红细胞(HYPO)、大血小板(LPLT)、原幼细胞(BLASTS)、红细胞大小不等(ANISO)为86.7%~100.0%,高色素性红细胞(HYPER)为69.2%,有核红细胞(NRBC)、变异淋巴细胞(ATYP)、血小板凝集(PLT-CLM)为13.3%~32.3%;以上所有参数真阴性率为82.8%~100.0%(平均95.0%)。结论仪器对报警参数的真阴性率高而有较强的筛检能力。仪器对NRBC、ATYP、PLTCLM的异常报警提示准确性不理想,而对其他参数的异常报警提示有较高的可信性。因此,对于结果有异常提示的标本应当配合手工镜检确诊。

【Abstract】 Objective To study the reliability of the function of ADVIA120 automatic hematology analyzer on abnormal shape of cells. Methods 160 samples with abnormal parameter hinted by the analyzer and 112 samples without hinted abnormal parameter were tested comparative with microscopic examination of blood smears after Wright’s staining. Results True positive rate: immature granulocyte(IG),hemoglobin concentration variation(HC-VAR),microcyte(MICRO),left shift(LS),macrocyte(MACRO),hypochromic erythrocyte(HYPO),large platelet(LPLT),blasts,anisocytosis(ANISO) 86.7%-100.0%,hyperchromic erythrocyte(HYPER) 69.2%,nucleated red blood cell(NRBC),atypical lymphocyte(ATYP),platelet clumps(PLTCLM)(13.3%)-32.3%.All the parameters above had a true negative rate of 82.8%-100.0%(mean of 95.0%). Conclusions The analyzer has a high true negative rate towards alarming parameter and has a strong screening power.The abnormal alarming message on NRBC,ATYP,PLTCLM produced by this analyzer is not accurate ideally,but the abnormal alarming message of other parameters is reliable to a great extent,therefore the samples with the outcome hinted abnormal should be compared with microscopic observation to find the diagnosis.
