阿奇霉素对慢性鼻窦炎鼻息肉患者术腔黏膜上皮化的影响

【摘要】 目的:观察阿奇霉素对慢性鼻窦炎鼻息肉患者术后术腔黏膜上皮化的影响。方法:随访59例行鼻内镜手术的慢性鼻窦炎、鼻息肉患者,患者术后均用布地奈德喷鼻,1月后头孢菌素组37例加用头孢丙烯500 mg,口服,每日1次,共3周;阿奇霉素组22例加用阿奇霉素500 mg,口服,每日1次,共3周;观察2组术腔黏膜上皮化的情况和时间。结果:随访6个月,头孢菌素组28例上皮化,9例在随访期间未上皮化;阿奇霉素组22例均上皮化,两组比较差异有统计学意义(χ2=4.573,P=0.033)。头孢菌素组22~126 d黏膜上皮化,平均(51.143±27.655)d,阿奇霉素组11~83 d上皮化,平均(33.046±18.874)d,两组差异有统计学意义(t=2.624,P=0.006)。结论:阿奇霉素治疗慢性鼻窦炎鼻息肉术后术腔黏膜慢性炎症、促进术腔黏膜上皮化有良好的疗效,可作为患者术后治疗的一种选择。

【Abstract】 Objective: To observe the effect of Azithromycin on the epithelialization of nasal mucosal of chronic sinusitis after endoscopic sinus surgery.Methods: From December 2004 to December 2005,59 patients with chronic sinusitis and/or nasal polyp were treated with endoscopic surgery.One month after the operation,they were divided into two groups,and 37 patients were treated with cephamycin as a control group and 22 with Azithromycin as a experimental group.The course and the number of epithelialization were counted.Results: Within 6 months,epithelialization happened in 22-126 days(average 51.1423±27.655 days) in control group,and in 11-88 days(average(33.046±)(18.874) days) in experimental group,and the difference was statistically significant(t=2.624,P=0.006).Epithelialization happened in 28 cases in the control group and 22 cases in experimental group,and the difference was also statistically significant(χ~2=4.573,P=0.033).Conclusion: Azithromycin is effective in accelerating the epithelialization of nasal mucosa after endoscopic sinus surgery.
