【摘要】 目的探讨带状疱疹后遗神经痛(PHN)伴发内脏反应的临床特点。方法分析25例带状疱疹后遗神经痛伴发内脏反应的临床表现、病程特征及诊疗经过,并追踪随访。结果25例的PHN均为脊神经损害,呈节段性分布,伴有感觉过敏带,8例(32%)有轻微脊髓横贯性损害;内脏反应为反复发作的功能性激惹症状,其中消化系18例占72%,泌尿系7例占28%,与PHN同时或随后发生,早期漏诊误诊21例(84%);以抗病毒为主的综合治疗有效,复发率低。结论PHN临床特征为胸段脊神经痛,呈节段性分布;伴发内脏反应常见为消化、泌尿系统激惹性症状,病程呈反复发作性经过;临床误诊漏诊率较高;本病预后良好。更多还原

【Abstract】 Objective To study the clinical features of post-herpetic neuralgia(PHN)associated visceral reactions. Methods To analyze the clinical manifestation, features of the course, diagnosis and treatment of PHN associated visceral reactions. Results 25 patients with PHN had segmented damage of apinal nerves, accompanied with hyperesthetic zone. 8 of these patients (32%) had minor transverse lesion of spinal cord. Recurrent irritation of visceral organs was the major symptoms. 18 patients (72%) showed the symptom associated with the alimentary system. The rest of the patients (28%) had the symptoms associated with the urinary system. The reactions took place simultaneously with or after PHN. 21 patients(84%)were misdiagnosed or not diagnosed at the early stage of disease onset. The combined therapy with anti-viral drugs was effective in disease management and the recurrence rate was low. Conclusion The clinical features of PHN were segmental thoracic neuralgia. The most often associated visceral reactions were the irritation of the alimentary and urinary system. The disease had a high recurrent rate and the rate of misdiagnosis was also high. Prognosis of PHN is good.更多还原
