ECLIA定量检测HBsAg及HBeAg与HBV-DNA相关分析
Analysis of correlation between quantitative detection of serum HBeAg and HBsAg by ECLIA and the content of HBV DNA
【摘要】 目的探讨电化学免疫发光法(ECLIA)定量检测HBeAg和HBsAg,并分析与DNA(HBV-DNA)含量之间的相关性,为临床提供诊断治疗依据。方法筛选乙肝病人标本500份,以荧光定量PCR法检测HBV-DNA(以HBV-DNA>500 copies/ml为阳性),按DNA含量不同分9组,同步以ECLIA检测HBeAb、HBeAg、HBsAg。结果HBeAg的含量(IU/ml)与HBV-DNA的含量(copies/ml)成正相关(r=0.99),在HBV-DNA 5×102~1×106copies/ml时,HBV-DNA的阳性率比HBeAg高(P<0.05),在HBV-DNA大于1×106copies/ml时,HBV-DNA的阳性率与HBeAg相比差异无显著性(P>0.05);HBV-DNA在500~1×106copies/ml之间,HBeAb有较高的阳性率,并随着HBV-DNA含量增加而逐渐下降。HBsAg的含量COI(cut off index)与HBV-DNA含量呈负相关(r=-0.94)。结论ECLIA的HBeAg定量检查可指示病人的传染性强弱,但尚不是最可靠的指标;HBeAg阴性、HBeAb阳性不是传染性消失的可靠指标;HBsAg的定量检测意义不大。
【Abstract】 Objective To analyze the correlation between quantitative detection of serum HBeAg and HBsAg by electrochemiluminescene immunoassay (ECLIA) and the content of HBV DNA and provide a laboratory indication for clinical diagnosis and treatment. Methods The sera from 500 chronic B hepatitis patients were collected and fluorescence quantitative PCR was used for detection of HBV DNA.Then the sera were divided into 9 groups based on HBV DNA levels,and HBeAb、HBeAg、HBsAg were simultaneously detected by ECLIA. Results The results showed:① the content of HBeAg(IU/ml) was positively correlated with the content of HBV DNA(copies/ml)(r=0.99).When HBV DNA level in sera was in the range of 5×10~2/ml~1×10~6 copies/ml,the positive rate of HBV DNA was higher than that of HBeAg(P<0.05);while there is no significant differences between HBV DNA positive rate and HBeAg positive rate when HBV DNA >1×10~6 copies/ml(P>0.05);②when HBV DNA level in sera was in the range of 5×10~2/ml~1×10~6 copies/ml,the positive rate of HBeAb was higher,and dropped gradually along with the increase of HBV DNA content;③the content of HBsAg(COI,cut off index) showed negative correlation with the content of HBV DNA(copies/ml)(r=-0.94). Conclusion Quantitative detection of HBeAg(IU/ml) by ECLIA can indicates the infection extent in the patients,but sometimes the detection may fail.Negative HBeAg and positive HBeAb are also not the reliable indicators for judging the disappearance of infectivity.
