【摘要】 目的探讨急性盆腔炎的发病特点及治疗方案。方法回顾性分析1993~2003年因急性盆腔炎住院治疗的285例患者的临床资料。结果患者平均年龄为33·99岁,发病主要诱因有近期流产、清宫等宫腔及宫颈手术、不规则阴道出血、劳累等。患者的主要症状包括下腹痛(94·4%)、发热(64·2%)、白带增多(33·0%);体征包括附件区压痛(90·9%)、子宫压痛(83·5%)、宫颈举痛(55·5%)、附件增厚(51·6%)和附件包块(17·2%)。应用广谱抗菌药物治疗的同时是否加用抗厌氧菌药物对患者的体温、血象恢复无影响(P>0·05)。结论急性盆腔炎好发于性生活活跃期妇女,子宫/附件区压痛及宫颈举痛或摇摆痛是诊断急性盆腔炎的必要体征。单纯应用广谱抗菌药物和广谱抗菌药物联合抗厌氧菌治疗效果无差别。更多还原

【Abstract】 Objective To explore the characteristics and treatment of acute pelvic inflammatory disease (PID).Methods The clinical datum of 285 patients with acute PID were retrospectively reviewed from 1993 to 2003 in the First Hospital of Peking University.Results The average age of 285 patients was 33.99.The main causes of PID included uterine cavity operations such as artificial abortion and curettage,irregular vaginal bleeding,tire and so on.Lower abdominal pain (94.4%),fever (64.2%),increasing vaginal discharge (33.0%) are the most common symptoms.90.9% of patients had adnexa uteri tenderness,83.5% with uterine tenderness,55.5% with cervix uteri motion tenderness.All the patients treated with antibiotics were divided into two groups by the sort of drugs.One group was treated with broad-spectrum antibiotics,the other was treated with broad-spectrum antibiotics combined with antibiotics against anaerobic bacteria.There was no significant difference between two groups in the period of temperature and blood constant restituting after the treatment(P>0.05).Conclusion Sex activity women are high-risk population for acute PID.Uterine/adnexal tenderness or cervical motion tenderness are essential body features to diagnose acute PID.The curative effects have no differences between broad-spectrum antibiotics only and broad-spectrum antibiotics combined with antibiotics against anaerobic bacteria.更多还原
